Treatment of any disorder of the salivary glands, neoplastic or nonneoplas
tic, must begin with a diagnosis of that lesion. Clearly, a failure to properly
treat a disease entity may stem form inadequate diagnosis of that disease
process. Such a diagnosis begins with the acquisition of a thorough history,
followed by the performance of a comprehensive physical examination,
the collection of routine and special radiographic data, and ultimately
the development of a differential diagnosis. It is the clinician s previous
experience and mental database that allow for the establishment of an
all inclusive differential diagnosis. Often, entities within the differential
diagnosis may be excluded as the surgeon appreciates various clinical and
radiographic nuances. As this article will show, a comprehensive differen¬
tial diagnosis may allow for appropriate and successful treatment of a
patient even without an incisional biopsy.

Diagnostic imaging can play an important role in the evaluation of a
number of disorders involving the major salivary glands. Imaging modal¬
ities frequently used in the evaluation of salivary disorders include plain
films, sialography, ultrasound, computed tomography, radionuclide im¬
aging, and magnetic resonance imaging. This presentation discusses the
role(s) of these different modalities in the evaluation of a variety of
inflammatory, metabolic, and neoplastic disorders involving the major
salivary glands.

Salivary gland infection has a varied etiology and may afflict all major or
minor salivary glands. Modern medical therapeutics, although decreasing
the incidence of severe or even fatal in patient sialoadenitis, has, ironically,
increased the occurrence of minor sialoadenitis seen in offices and out¬
patient facilities. Surgical intervention, although less often necessary in the
antibiotic era, must remain part of the surgeon s armamentarium for the
recalcitrant or recurrent salivary gland infection. New data on the flora of
sialoadenitis continues to evolve, as do newer therapies to treat it.

After several hundred years, we are now in a position to identify the
seemingly innocuous sublingual gland as the origin of much suffering and
morbidity over the many years since it has been classified and diagnosed.
This article was designed to discuss the origin, pathogenesis, differential
diagnosis, and treatment modalities for intraoral and cervical mucous
escape pseudocysts. It is hoped by the author that less invasive and less
mutilating surgery will be performed to eradicate the lesion arising from
the continuously self excreting salivary gland whose physiology is quite
different from other major salivary glands and whose role in the etiology
of pseudocysts of the oro cervical region has defied the experts of the ages.

and suggested management. Particular attention is directed toward the
evaluation, diagnosis, etiology, and surgical management of sialolithiasis.
Incisional Parotid Biopsy for Diagnosis of Systemic Disease 505

Incisional biopsy of the parotid gland is a diagnostic biopsy indicated to
confirm suspected systemic disease involving the parotid such as Sjogren s
syndrome, sarcoidosis, sialosis, benign lymphoepithelial lesions, HIV par¬
otitis, and lymphoma. It is a straightforward procedure that can be accom¬
plished in the office. It has low morbidity, few complication, and a far
superior yield to minor salivary gland biopsies.

The subjects of histogenesis and the classification of salivary gland tumors
contain important concepts for the surgeon treating patients with benign
and malignant salivary gland tumors. An appreciation for the classification
of salivary gland tumors results in the surgeon rendering therapy based
on a prediction of their biologic behavior. Along with good interaction
with the pathologist processing the specimen, an understanding of these
concepts allows for proper care of patients with salivary gland neoplasms.

The parotid gland is the site of 80% of epithelial salivary tumors. The
majority (80%) are benign and are best dealt with surgically by superficial
or total parotidectomy depending on their location. Malignant tumors are
uncommon and have a diverse histopathology varying from low grade to
very aggressive. Treatment of these lesions will depend upon staging,
histology, involvement of the facial nerve, and other factors. Although
surgery is the modality of choice, lesions with adverse prognostic factors
benefit from postoperative radiation therapy. Inoperable lesions may be
palliated with radiation.

The smaller the salivary gland becomes, the more likely that the tumor
is malignant. Following histological diagnosis and appropriate imaging,
treatment is generally surgical. Because of the confined space in the floor
of the mouth and the proximity of important structures, radical surgery
has the potential for considerable morbidity and implications for recon¬
struction.

benign disease as being essentially of two types and discusses malignant
disease in the context of six major classifications. The elements of histogen
esis, differentiation, diagnosis, and treatment are reviewed.

Salivary gland diseases in children are uncommon. The salivary gland
disorders that affect children encompass the entire spectrum of inflamma¬
tory, obstructive, and neoplastic diseases. However, the incidence, presen¬
tation, course, and severity of these diseases in children often vary greatly
from that in adult patients. An overview of the most common inflamma¬
tory, obstructive, and neoplastic diseases is presented.
