A transformational strategy for leadership and organization of
cardiovascular services was designed by the nursing and physi¬
cian leaders of the Cardiovascular Department at Children s Hos¬
pital in Boston. The new Cardiovascular Program included
changes in governance, administration, and clinical operations.
This organizational innovation is discussed and the successful
outcomes achieved to date are described. The level of creativity
and commitment demonstrated by all staff is highlighted.
Restructuring: An Integrated Approach to Patient Care 171

This article describes two generations of restructuring at Chil¬
dren s Hospital (Boston) over a 6 year period that have resulted
in the Integrated Care Delivery Model. It also provides an
overview of the concepts involved in: restructuring, working
with external consultants, role descriptions, delegation, and im¬
plementation, as well as discussing the challenges and rewards of
developing such a model.

In response to environmental changes brought about by the de¬
bate over health care reform, hospitals are seeking means to de¬
crease costs while improving quality. By examining the process of
health care delivery, problems in the system can be highlighted
for further investigation. Development of clinical practice guide¬
lines by an interdisciplinary team can be an effective approach to
decrease undesirable practice variation, standardize appropriate
resource use, and measure the effectiveness of care through de¬
fined expected patient outcomes. By ensuring the appropriate, ef¬
ficient, and effective delivery of health care, savings in cost can be
realized by the elimination of duplication, rework, and waste.

Peer review is a major component of a professional practice and
shared governance model in which professional staff nurses are
given responsibility for practice and patient outcomes. Careful
planning is necessary for the development of a peer review pro¬
gram. Components include definition of roles, responsibilities,
and performance standards, as well as the development of an
evaluation tool and process. Peer review affords nurses many op¬
portunities for growth and development and allows them to de¬
velop communication, evaluation, and presentation skills. The
concept of peer review may expand to meet the needs of chang¬
ing health care delivery models.
The Role of the Cardiovascular Social Worker 211

The clinical social worker is an integral component of the Inte¬
grated Care Model used in the Cardiovascular Program at Chil¬
dren s Hospital (Boston). This article provides the reader with an
understanding of the social worker s role with families and as
part of the interdisciplinary team. Case examples are used to il¬
lustrate the key components of clinical social work practice, in¬
cluding crisis intervention, solution focused treatment, and grief
counseling. The importance of maintaining sensitivity to cultural
differences in working with families is discussed.

Providing seamless care across the continuum is becoming an in¬
creasingly challenging task. A new role, the patient care coordina¬
tor (PCC), has been established at Children s Hospital (Boston) to
meet this challenge. This role combines the functions of discharge

planning and utilization management along with a strong educa¬
tional component. By collaborating with interdisciplinary team
members, payers, and community providers, the PCC facilitates
continuity of care for patients in the Cardiovascular Program.

Parent participation is essential in the care of hospitalized chil¬
dren. The role parents play varies throughout the continuum of
hospitalization. This article reviews the evolving role of parents
and highlights three programs specifically designed to support
the philosophy of family participation and partnership.

lines for dyslipidemias in the pediatric and adolescent popula¬

Current Management of Congestive Heart Failure in

Congestive heart failure is a pathophysiologic clinical syndrome
in which the heart is unable to pump an adequate amount of
blood to meet the metabolic needs of the tissues. The manage¬
ment of infants and children with heart failure is multifaceted
and includes measures to improve cardiac output by altering pre¬
load, afterload, and contractility. Nursing management includes
measures to decrease cardiac workload and promote normal
growth and development. This article reviews the pathophysiol
ogy of congestive heart failure and outlines current therapeutic
management in infants and children.

Infection with human immunodeficiency virus (HIV) has become
a major pediatric health concern in the United States and around
the world. Pediatric HIV infection is a multisystem illness that
presents an ongoing challenge to practicing nurses. Most clinical
cardiovascular diseases in children with HIV have been underre
ported, and often are clinically occult. The preclinical detection of
cardiovascular abnormalities results in early therapeutic inter¬
ventions and reduces cardiovascular morbidity and mortality.
Cardiovascular involvement is important in the natural history
and prognosis of HIV infection.

ular outflow tract (LVOT) obstruction and aortic stenosis across
the lifespan. The history of conduits, homografts, and valves is
detailed with a focus on the pulmonary autograft procedure. Per¬
formances and comparisons of valve substitutes also are dis¬
cussed.

going an interventional cardiac catheterization. New catheters,
monitoring equipment, and standards for skilled personnel have

Pediatric Cardiac Surgery 347

The successful early repair of congenital heart disease has placed
the focus on the long term outcome for surviving patients. This
article reviews the neurologic risks of cardiopulmonary bypass,
deep hypothermia, and circulatory arrest. Potential neurologic
and developmental sequelae in both the immediate postoperative
period and the long term are discussed, with a review of current,
research based literature.

Cardiac arrhythmias are being recognized with increasing fre¬
quency in the pediatric population. This article describes com¬
mon causes of pediatric arrhythmias as well as discussing hemo
dynamic assessment and diagnostic testing, with an overview of
specific arrhythmias.
Inhalation of Nitric Oxide: An Innovative Therapy for
Treatment of Increased Pulmonary Vascular Resistance 381
Skye Rykerson, John Thompson, and David L. Wessel
Critical care of patients during pulmonary hypertensive crises is
difficult and treatment is hazardous. Conventional therapy has
relied on hyperventilation with oxygen and the use of nonselec
tive intravenous vasodilators. The limitations of this approach
have prompted clinical investigations of new therapeutic modali¬
ties based on recent advances in vascular biology. Inhaled nitric

oxide can be used selectively to manipulate the pulmonary vas
culature and treat a variety of pulmonary hypertensive disorders.
Preliminary studies appear promising as results of properly con¬
ducted trials to evaluate efficacy and toxicity are awaited.
