Advances in computed tomography technology and computer
science have allowed dramatic improvements in the ability to
image the abdomen. This article reviews the specific technical
advances in CT scanning and those applications that are clinically
relevant for the gastroenterologist.

Transjugular intrahepatic portosystemic shunt (TIPS) has rapidly
become a widely disseminated procedure. It is both feasible and
safe. TIPS has proved effective for treatment of acute variceal
bleeding and is probably an effective therapy for intractable asci
tes. Nonetheless, it is important to define the appropriate subpop
ulation best suited for TIPS. Given the poor long term patency of
TIPS and the need for close follow up, TIPS is not optimal therapy
for an unreliable patient who is not a candidate for liver trans¬
plantation. TIPS is especially useful in the patient anticipating
liver transplantation soon after TIPS. Further study is needed to
define the optimal use for TIPS.

Magnetic resonance cholangiography offers a noninvasive
method of obtaining images of the biliary system without the use
of a contrast agent. The diagnostic accuracy of MR cholangiogra¬
phy approaches that of direct cholangiography, suggesting that
in some patients MR cholangiography might be used either in¬
stead of endoscopic retrograde cholangiography or percutaneous
transhepatic cholangiography or to direct therapeutic techniques.
When combined with conventional MR imaging, MR angiogra
phy, MR spectroscopy, and perhaps MR guided biopsy, a com¬
prehensive workup of the patient with hepatobiliary disease is
possible in approximately 1 hour.

Imaging studies after laparoscopic cholecystectomy are done if
postoperative complications are suspected. Bile duct injuries re¬
quire cholangiography to classify the injury and to exclude other
problems such as cystic duct remnant leak or choledocholithiasis.
Computed tomography and sonography are good screening ex¬
aminations for suspected abscess, hematoma, or bowel perfora¬
tion.

The double contrast upper gastrointestinal series makes medical
and economic sense as a cost effective alternative to endoscopy
for evaluating patients with dyspepsia or other upper GI symp¬
toms who fail to respond to an empiric trial of medical therapy.
These studies are capable of detecting most clinically significant
disease in the upper GI tract. Data indicate that double contrast
studies also can achieve a high sensitivity in the diagnosis of
malignant lesions without exposing an inordinate number of pa¬
tients to unnecessary endoscopy.

length, the changing location of its many overlapping coils, and

The swallowing tract extends from the lips to the gastric cardia.
The barium swallow provides a global view of oral, pharyngeal,
and esophageal motility and pharyngeal and esophageal mor¬
phology. Barium pharyngography is the best test to assess the
functional capabilities of the pharynx. The barium swallow may
be used as the sole diagnostic examination or as a guide to what
procedure or intervention should be forthcoming.

Patients with acute inflammatory and infectious bowel disease
require prompt diagnosis and management to relieve their symp¬
toms and to minimize potential complications. Although defini¬
tive diagnosis of infectious or idiopathic bowel disease ultimately
rests on histologic and bacteriologic documentation, radiologic
studies can play a crucial role in determining the initial site and
extent of intestinal involvement, therapeutic response, and fol¬
low up of complications. This article reviews the radiologic ap¬
proach to patients with acute enterocolitis and provides differen¬
tial diagnostic guidelines.
Endoscopic Ultrasonography of the Gastrointestinal Tract 385

Endoscopic ultrasonography (EUS) combines the advantages of
conventional endoscopy with the capabilities of ultrasonography.
EUS allows clinicians to see through the wall of the gastrointesti¬
nal tract. The close proximity allows the use of relatively high
frequencies with the resulting increase in tissue contrast and
resolution. Despite some limitations, this new modality does open
new horizons in the diagnosis and characterization of upper
gastrointestinal diseases.

As the AIDS epidemic continues to grow, it becomes increasingly
important that physicians are aware of the gastrointestinal and
abdominal diseases specific to this group of patients. Most AIDS
patients exhibit gastrointestinal symptoms at some time during
the course of their disease. Clinical AIDS is often determined by
identifying an opportunistic infection or neoplasm of the gastroin
