There are many ways to gain access to the chest. In recent years,
there has been a rediscovery of the clamshell incision, an evolving
concept of the utility incision for video assisted thoracic surgery
(VATS), and a continued emphasis on the importance and use¬
fulness of the muscle sparing axillary thoracotomy, which contin¬
ues to be the author s most frequently used incision.
Mediastinal Lymph Node Dissection for Lung Cancer:

A mediastinal lymph node dissection as opposed to lymph node
sampling should be an integral part of the operation in all pa¬
tients with resectable lung cancers. A systematic mediastinal node
dissection affords the most accurate staging and a long term
survival in some patients when the positive regional nodes are
also removed. It is safe and easy to do, adds only 20 to 30 minutes
to the surgical procedure, and morbidity is minimal. Our reported
survival data have been based on the routine use of mediastinal
lymph node dissection in conjunction with pulmonary resection.
Techniques of lymph node dissection for both right and left
sided tumors are described along with a historical background of
their evolution.

Segmental Resection 205

Following a short historical note, the operative technique of ana¬
tomic resection of individual bronchopulmonary segments is de¬
scribed. Segmentectomy is suited ideally to the surgical manage¬
ment of inflammatory lesions and pulmonary metastases. The
high local recurrence rate makes it unsuitable for the treatment
of primary lung cancer, except in patients with severely compro¬
mised pulmonary function.
VATS Lobectomy with Mediastinal Lymph Node Sampling

With a thoracoscope for visualization, a lobectomy with mediasti¬
nal node dissection can be accomplished with standard thoracot
omy instruments through a 6 cm incision without spreading the
ribs. This article describes the indications, contraindications, and
technical details of this procedure.
Sleeve Lobectomy 233

Sleeve lobectomy is the alternative procedure to pneumonectomy.
It can be an operation of choice for lung cancer, if technically
feasible, and is the ideal procedure for low grade pulmonary
malignancies and inflammatory bronchial strictures. Precise ra
diologic bronchoscopic assessment defines the need and probabil¬
ity for sleeve lobectomy. Close adherence to precise technical
principles will achieve excellent clinical results with a low mor¬
bidity and mortality.
Pneumonectomy 253

Success in pneumonectomy, or any pulmonary surgery, de¬
pended upon learning the principles of positive pressure ventila¬
tion with endotracheal intubation, management of the pleural
space, and confirming the advantage of anatomic dissection in
individual ligation of hilar structures. The earliest successful
pneumonectomies were performed for suppurative disease, but
eventually became the procedure of choice for treatment of lung
cancer until it became known that lobectomies would result in
the same cure rate but with a decreased complication rate. At the
present time, pneumonectomy is reserved for patients whose
cancers require complete pneumonectomy for cure and for se¬
lected suppurative problems. Accepted techniques for conven¬
tional, intrapericardial and extrapleural pneumonectomies are de¬
scribed along with indications for the particular procedures.

In patients with drug resistant Mycobacterium tuberculosis and
localized disease, pulmonary resection is indicated followed by
continued medical therapy. In our series of pulmonary resection
for tuberculosis, pneumonectomy was required in more than 50%
of the patients. Extrapleural pneumonectomy is the preferred
approach and can be performed with a mortality rate of less
than 5%.

Occasional tumors arising in the upper lobes or in the origin of
either main bronchus are localized sufficiently to permit curative
resection by sleeve pneumonectomy. Operative mortality is sig¬
nificant, but it can be lowered by careful attention to surgical and
anesthetic techniques.

cus tumors—the classic Shah Paulson posterolateral approach,
the modification as recently described by Dartevelle, and a hemi

This article discusses technical approaches to the pulmonary ar¬
tery, as well as techniques used in the pulmonary arterioplasty.
Although pulmonary artery reconstruction can be performed now
with minimal perioperative morbidity and mortality, the long
term survival rate is still disappointingly low, chiefly because of
the development of distant metastases.
Technique of Superior Vena Cava Resection and

Superior vena cava (SVC) resection and reconstruction is a techni¬
cally demanding procedure associated with several concerns.
During operation, continuous arterial and venous pressure mea¬
surements are essential to monitor and maintain the physiologic
arterial venous brain parenchymal gradient. Technical details mit¬
igating this hemodynamic compromise include intraluminal

shunt procedures, pharmacologic agents and fluid implementa¬
tion, short venous clamping time, and anticoagulation therapy.
SVC replacement is usually afforded using a normal or ringed
polytetrafluoroethylene graft.
Pericardial Drainage Procedures 359

Various techniques for pericardial drainage are described, each
with strengths and limitations. No single technique is optimal for
all circumstances. In most cases, subxiphoid drainage offers an
excellent combination of safety with immediate and long term
efficacy. In addition, many patients present with pericardial tam
ponade which poses special problems in management. Pericardial
sepsis may require more aggressive treatment, particularly with
Haemophilus influenzae.
