This article provides an overview of the use of revascularized tissue
transfer in the reconstruction of craniomaxillofacial defects. General recon¬
structive principles and considerations in patient selection are discussed.
Specific defects are divided into general anatomic regions of the mandible,
midface, skull base, and massive soft tissue defects. Flap selection and
dissection for these regions are discussed and illustrated with specific
cases.

The subcranial approach represents a recent advance in craniofacial and
anterior skull base surgery. This article describes the subcranial approach
as applied to the management of both congenital and acquired craniofacial
anomalies.

Plate osteosynthesis has become an indispensable component in cranio¬
maxillofacial surgery in the treatment of fractures and osteotomies. A new
titanium miniplate system, which allows three dimensional stabilization
using only little osteosynthesis material, is introduced. The smallest com¬
ponent of this system, an open cube or square stone, permits easy
adjustment, stability against torsion forces, and better blood supply to the
bone fragments.

Many different methods of repair of the unilateral cleft lip deformity have
been described. The Millard rotation advancement technique for repair of
the unilateral cleft lip is a flexible and versatile technique that is used
widely. This article recognizes the use of this excellent technique and
describes several small modifications. These technical modifications intro¬
duce increased predictability and describe a method to prevent scar con¬
traction across the vermilion border of the upper lip.

The unilateral cleft nasal deformity is a difficult aesthetic and functional
deformity to correct surgically. If addressed at the time of primary lip
repair, the asymmetries associated with the unilateral cleft lip deformity
can be eliminated. This can alleviate the need for intermediate rhinoplasty.
The initial nasal deformity associated with the unilateral cleft lip is de¬
scribed. A detailed explanation of the technique used to repair the primary
unilateral cleft lip nasal deformity is discussed.

Orthognathic surgery is considered a safe surgical procedure for improv¬
ing facial aesthetics. Correct preoperative planning is of foremost impor¬
tance for stable long term results. The planning procedure, including the
cephalometric prediction and the model surgery, is discussed. The various
osteotomies of the mandible and maxilla commonly used are presented,
and the resultant soft tissue changes in orthognathic surgery are summa¬
rized.

Fractures of the middle third of the facial skeleton should be thought of
as craniozygomaticomaxillary fractures and not simply as malar or Le Fort
fractures. Restoration of the occlusal relationship or the position of the
malar prominence requires the precise three dimensional reconstruction
of one or both zygomaticomaxillary buttresses within the midfacial skeletal
unit. This is best accomplished by using extended access approaches that
allow for direct evaluation of bony realignment and rigid internal fixation
devices (plates and screws) that provide greater stability to the fracture
reductions.

The management of patients with cranio orbital injuries presents a chal¬
lenge to reconstructive surgeons. The injuries result from high velocity
forces to the lateral and central regions of the craniofacial skeleton. Recent
advances have led to improved cosmetic and functional results. The sub
cranial approach allows early definitive treatment of the skull base and
facial fractures including optic nerve decompression in a one stage proce

dure without the standard transfrontal approach and craniotomy with
frontal lobe retraction. The technique of symmetric centripetal compression
for the correction of traumatic telecanthus injury is described. A brief
summary of the results obtained in 505 patients with frontonaso orbital
injuries and 1025 patients with zygomatic complex injuries is given.
