The importance of cooperation and understanding between the
obstetrician and anesthesiologist in the management of obstetric
emergencies cannot be overemphasized. Indications for cesarean
section and choice of anesthetic are significant considerations.
Algorithms for the management of special treatment problems
such as malignant hyperthermia, postdural headache, and epi
dural abscess and hematoma are presented. Posting of protocols
for the management of particular crises in both delivery and
postanesthesia care units is strongly recommended.
The Antepartum Patient: Sudden Changes
in Central Nervous System Status and Sensorium 13
Bruce A. Work
An approach to the pregnant patient with altered consciousness
is presented. The unconscious pregnant patient often represents
a straightforward simple problem if the event is a minor syncope
secondary to physiologic changes of pregnancy. Most loss of
consciousness is postictal in nature after either an epileptic or
eclamptic seizure. If these are ruled out, other more serious
causes must be considered, and the patient must be fully evalu¬
ated by appropriate neurologic consultants.

Sudden Chest Pain and Cardiac Emergencies in the
Obstetric Patient 19

The differential diagnosis and work up of a patient with chest
pain during pregnancy is presented in this article. This is fol¬
lowed by discussions of cardiac emergencies including hyperten¬
sive crisis, pulmonary edema, arrhythmias, cardiopulmonary re¬
suscitation, myocardial infarction, and aortic dissection.

Acute pulmonary dysfunction and distress is not uncommon in
the obstetric patient. In these situations, however, a quick assess¬
ment and intervention is necessary in many cases. This article
familiarizes the practicing obstetrician with the physiology and
therapy to evaluate and effectively treat these patients.

The anatomic and physiologic changes of pregnancy make the
diagnosis and management of abdominal pain difficult. Abdomi¬
nal conditions both related and unrelated to the pregnancy may
be life threatening for mother or fetus; therefore, accurate diagno¬
sis and proper management are essential. This article reviews the
most common conditions presenting as acute abdominal pain
during pregnancy.

This article covers the major physiologic and pathologic findings
to be considered when managing pregnant patients who have
experienced moderate to severe trauma during gestation. In¬
cluded are discussions related to physical, radiologic, and labora¬
tory assessment, as well as techniques to ascertain fetal well
being.
Acute Hypotension Related to Sepsis in the Obstetric Patient 91

Serious bacterial infections resulting in septic shock are rare dur¬
ing pregnancy. If prompt evaluation and therapy are not pro¬
vided, however, serious maternal morbidity and mortality may
occur. Empirical broad spectrum antimicrobial therapy should be
instituted early, and cardiovascular insufficiency should be
treated with aggressive volume expansion. Hypotension refrac¬
tory to volume expansion may require inotropic supportive
therapy. Future management will likely include a cocktail of

cytokine antibodies or antagonists directed at controlling the
pafhophysiologic derangements associated with systemic activa¬
tion of these immunomodulators.

Acute hypotension in the setting of obstetric hemorrhage most
often signifies abrupt and dangerous blood loss. Management
involves the prompt reconstitution of circulating volume and
replacement of lost blood. Simultaneously, the treating physician
must actively search for and eliminate the source of bleeding.
Successful management hinges upon an organized team approach
with appropriate input from relevant specialties.
Management of Drug Dependency, Overdose, and
Withdrawal in the Obstetric Patient 131

This article discusses the approach to pregnant patients with
substance use problems, a growing problem in obstetric practice.
Two alternatives used to detect patients with problematic sub¬
stance use are discussed: (1) historic screening, which depends
on the patient to self report about her substance use; and (2)
toxological assays for substances or their metabolites in the
patients body fluids. Management of women using cocaine and
alcohol is also discussed.

Diabetic ketoacidosis in the obstetric patient is a medical emer¬
gency. Prompt diagnosis and treatment of this condition are
paramount to minimize adverse maternal and fetal outcome. The
clinician is provided guidelines for the management of the preg¬
nant diabetic patient who presents with diabetic ketoacidosis.
General therapeutic considerations and specific treatment regi¬
mens are discussed.
New Onset Hypertension in the Pregnant Patient 157

Hypertensive disorders are the most common medical conditions
complicating pregnancy. The assignment of the hypertensive dis¬
orders to specific categories is important because the treatment
and complications for the mother and fetus vary widely. Diagnos¬
tic and treatment regimens can be subdivided into antepartum,
intrapartum, and postpartum times of onset. Management deci¬
sions vary from outpatient observation to in hospital treatment
with magnesium sulfate and intravenous antihypertensive agents.

A high index of suspension and a thorough evaluation with
continual maternal and fetal surveillance are essential for optimal
maternal and fetal outcome.
Sudden Joint and Extremity Pain in Pregnancy 173
Curtis L. Lowery
To diagnosis sudden joint and extremity pain in pregnancy, the
obstetrician must call upon information from several disciplines,
including obstetrics, internal medicine, neurology, and orthope¬
dics. The initial evaluation of the patient should begin with a
generalized assessment, and more distant causes of the pain
should be ruled out as possible causes are considered, evaluated,
and rejected. This article discusses the importance of medical
history, physical examination, laboratory studies, synovial fluid
analysis, and immunologic studies to aid in the diagnosis of
pain, as well as common presentations of musculoskeletal pain
in pregnancy.
