The Chemistry of Natural Rubber Latex 1

Natural rubber latex consists of particles of rubber hydrocarbon
and nonrubber substances dispersed in an aqueous serum phase.
The nonrubbers, amounting to more than 5%, include proteins,
carbohydrates, lipids, and inorganic salts. Latex concentrate is
obtained by centrifugation of latex, and dry rubber is obtained
by acid coagulation, creping, size reduction, and drying. Rubber
products, such as tires, and latex products, such as gloves and
balloons, are made by mixing sulfur, accelerator, and other com¬
pounding ingredients into the rubber or latex, followed by a
shaping operation, and finally by heating to vulcanize the rubber.
Latex products need to be leached thoroughly to reduce the
extractable protein content to low values.

Hypersensitivity reactions to rubber products are being reported
with increasing frequency. This article provides a chronological
review of cases appearing in the literature involving hypersensi¬
tivity reactions to latex, including contact dermatitis, urticaria,
and anaphylaxis. In addition, the authors describe methods cur¬
rently in use for the diagnosis of latex allergy.

Immediate type allergic reactions to latex produce signs and
symptoms typical of allergic reactions provoked by other aller¬
gens, including pruritus, erythema, and edema. Although the
skin is the most commonly affected organ, the eyes, nose, mouth,
lungs, heart, and gastrointestinal and reproductive tracts can all
be involved. Anaphylaxis is the most severe form of immediate
reaction. Chemicals added to latex during processing and perhaps
some chemicals added to glove powders may act as haptens and
produce delayed contact dermatitis. Typical manifestations of
contact dermatitis are erythema, cracking, fissuring, scaling, and
vesicles or blisters. The manifestations of immediate and delayed
allergic reactions must be differentiated from similar symptoms
produced by other agents and mechanisms.

Latex allergy caused by natural rubber latex proteins from Hevea
brasiliensis is recognized with increasing frequency due to the
increased exposure to latex products in recent years. The allergens
are not yet identified or fully characterized. There are no dependable
diagnostic methods that can be safely used to determine latex al¬
lergy. This article summarizes current knowledge about latex pro¬
teins and their allergenicity. Suggestions are also made to direct
• future research, both for understanding latex allergic mechanisms
and diagnosis of latex allergy.

Several in vivo and in vitro tests can be used to measure the latex
allergen contents of medical or consumer rubber products. In
particular, disposable rubber gloves contain widely variable lev¬
els of latex allergens and may pose major threats to latex sensitive
patients and health care workers. More efforts must be made to
reduce the allergen contents of medical gloves.

In Europe, dermatologists reported the first cases of contact urti¬
caria from rubber latex gloves 15 years ago. Knowledge of imme¬
diate hypersensitivity reactions to natural rubber latex (NRL) has
spread rapidly, and patient series have been reported from many
European countries. In addition to allergic reactions from
surgeon s gloves in patient care, attention has been paid to the

important occupational aspect of NRL allergy and how to prevent
sensitization of glove using health care workers and non health
care workers. A recent finding is the frequent occurrence of NRL
allergy in atopic children and the cross reactivity between NRL
allergens and certain food allergens. In Europe, diagnosis of NRL
allergy is currently based on skin prick testing. Several allergens
have been identified by immunoblotting and immunoelectropho
retic methods in gloves, pacifiers, and other manufactured rubber
products. Progress has also been made in the characterization of
some significant NRL allergens at the molecular level and in the
development of more standardized methods for diagnostic use.

A brief historical description of the development of the latex
and disposable medical glove industry is presented. Methods of
harvest, processing, compounding dipping, and postprocessing
are described with regard to the potential effect(s) on the biocom
patibility of the final product. Historical records, scientific princi¬
ples, and a number of theoretical explanations are set forth in a
cumulative challenge proposal for the increased incidence of Type
I hypersensitivity.

characteristics of glove barriers, and the differences between the
barrier integrity versus the effectiveness of glove barriers. The
literature is also reviewed followed by a discussion of current
glove standards and the application of gloves for clinical practice.

Although much has been learned about the immune mechanism
of latex allergy, specific proteins involved, and risk groups, little
is known about prevention and long term outcome of patients
with latex allergy. Many questions exist regarding the persistent
exposure of sensitized health care workers to latex. A uniform
approach to care of the patient with latex allergy, especially those
at risk in early childhood, is necessary until we know how to
prevent the development of latex allergy. This involves an effort
on behalf of hospitals, clinics, communities, and manufacturers
to reduce exposure to those at risk of sensitization as well as the
development of environments free from latex protein for those
who are already sensitized. An approach for this task is included
in this discussion, hi the past, many people were symptomatic
but went undiagnosed and without help. With a uniform ap

proach, we should be able to improve our patient outcome and
allow them to lead happier, productive lives.
Educational Challenges of Latex Protein Allergy 159

The emergence of latex protein allergy as a serious medical and
occupational condition has underscored the need for establishing
a medical education network between health care workers, gov¬
ernment regulatory agencies, and industry. The lack of such a
network and resulting haphazard communication have failed to
convince surgeons and hospitals of the gravity of this problem.
This article traces the short history of this disorder, the potential
financial impact in terms of costs to the health care profession,
and health care workers affected by latex protein allergy. Finally,
a proposed protocol for communication of such important infor¬
mation, based upon a model program implemented by the au¬
thor, is detailed.
