Diagnosis of disease often requires a tissue biopsy. This article dis¬
cusses the features of a proper biopsy from both a philosophic and
a practical viewpoint. The qualities of an adequate biopsy, the ap¬
propriate methods of tissue handling, the pros and cons of tissue
biopsy versus fine needle aspiration, and the use of frozen section
are described. The appropriate content of a surgical pathology re¬
port is also delineated.

The establishment of an accurate histologic diagnosis is extremely
important in the management of patients with head and neck le¬
sions. This article describes various biopsy techniques and discuses
the clinical implications in relation to common lesions involving
thyroid and salivary glands and cervical lymphadenopathy. The
diagnostic pitfalls and pros and cons of needle biopsy are also dis¬
cussed in detail.

three most useful biopsy techniques for thoracic lesions. Minimally

invasive procedures, such as mediastinoscopy and thoracoscopy,
must readily be available. As biopsy techniques and the lesions to
be biopsied become more complex, the surgeon should make the
decision regarding the particular technique. Exploratory thoracot
omy remains the ultimate thoracic biopsy.

Current breast biopsy techniques should recognize the changing
presentation and management of breast cancer. Nonpalpable le¬
sions detected and localized by mammography are common, and
the proportion of women suitable for and desirous of breast con¬
servation is increasing. Careful evaluation of the features of the
excised cancer and sophisticated decisions are to be emphasized,
not spared, as there are a wide variety of suitable treatments avail¬
able.

Barrett s esophagus are proposed and appropriate management
strategies of this condition are examined in depth.

The diagnosis of primary retroperitoneal masses is approached
best by CT scan guided transretroperitoneal core biopsy or by open
biopsy. Excisional or wedge biopsy is the preferred method for
undiagnosed lymphadenopathy. Resection without biopsy is
indicated for large or enlarging nonfunctional primary adrenal
masses.
