Preface
This book grew out of the authors’ collaboration at Boston University, while
teaching courses focused on the interaction between cinema and psychology.
Ingmar Bergman has said that film “should communicate psychic states”; we
came to believe that such depictions can be evaluated in the light of relevant
psychiatric theory.
We chose to concentrate on major feature films that portray madness in all its
many forms. This work is offered as an initial attempt to systematically approach
the relationship between psychiatric and cinematic images of madness. As an
introduction to the interaction of cinema and clinical psychology, the book as-
sumes that the reader has no formal background in either discipline. At times
the relationships between psychiatric and cinematic images of madness seem
direct, as when a psychiatrist evaluated a film or when a screenwriter developed
a case study. But more often, the relationships are very subtle and highly infe-
rential. To bring order into this chaos, we identify nine major themes: Society
and Madness; Possession as Madness; Eros and Madness; Murder and Madness;
War and Madness; Drugs and Madness; Paranoia and Madness; Sanity as
Madness, Madness as Sanity; and Madness and the Psychiatrist. For each theme
we contrast an early film with a more contemporary one, in the hope of tracing
the origins and underlying themes of different perspectives on insanity.
The first part, by Michael Fleming, explains the direct and contextual psychi-
atric and sociopolitical forces affecting the cinematic imagery of madness. The
second part, by Roger Manvell, presents the films in terms of their plot evolution
and their technical and historical components. Though many ideas were shared,
each part represents the sole views of its author.

The Depletion of Insanity in
the Feature Film
MICHAEL FLEMING and
ROGER MANVELL
This hook presents the first fully re-
searched assessment of the American
and European cinema’s portrayal of in-
sanity in key feature films over half a
century. In addition, it relates the very
varied interpretations of madness in
these films to the clinical theory and
practice of their period. The more nota-
ble films have been the subject of techni-
cal discussion by psychologists, psychia-
trists, and clinicians in their specialist
journals, and this body of critical evalua-
tion, virtually unknown to film critics
and historians, has been drawn upon to
give additional valuable insight to the
discussion of the films involved.
People with disturbed minds have fea-
tured as central characters in Greek clas-
sical tragedy, the Elizabethan-Jacobean
theater, and the Gothic horror novel of
the eighteenth and nineteenth centuries.
The modern twentieth-century cinema
has continued this tradition. Several
hundred feature films have turned on in-
sanity since The Cabinet o/ Dr Ccili^czri
first appeared in post-World War 1 Ger-
many some 5 5 years ago, and raised crit-
ical controversy concerning its treatment
of madness on screen. Ivlajor directors as
varied as Lang, Pabst, Hitchcock, Hus-
ton, Truffaut, Altman, Lumet, Kubrick,
Chabrol, Eellini, BuTTuel, Bergman,
Polanski, Eassbinder, Roeg, and Herzog
have made films about characters whose
minds were in one way or another se-
verely disturbed. Among these have
been such notable films of the past and
the present as A ejuirre, II die tic Jour,
Juliet of the Spirits, One Flew Over the
Cuckoo*s 2 lest, Repulsion, The Story of
y dele //., and Three Trices of Eve. All
these and many other films, both well
known and less known, receive detailed
consideration in this book, not only as
films, but from the clinical standpoint of
their time.
In the first part of the book, Professor
Fleming defines and discusses in detail
the major themes of madness that have
been presented in film, such as the family
and madness, possession and madness,
Eros and madness, murder and madness,
madness and the psychiatrist. All these
themes are presented in terms of their
interaction with period psychiatric
theory and practice. In the second part,
Professor Manvell provides notes, sy-
nopses, and critical comment on some
150 films dealing in one way or another
with the problems of insanity. The films
are selected either because of their out-
standing artistic and technical «quality, or
because, even if less artistically impor-
tant, they nevertheless have significance
for their handling of the subject. Profes-
sor Manvell also provides an appendix
that discusses briefly how madness was
treated in classical Greek and Roman
tragedy and especially in the Eliza-
bethan-Jacobean drama.
The book as a whole shows how our
society’s ideas and beliefs about
madness have been expressed in that
medium of our times, the film. It pro-
vides insight into our beliefs and con-
cepts about madness, if not our under-
standing of it.
About the Authors
Michael Fleming has been a professor of psychology at Boston Univer-
sity since 1973. He has published numerous articles dealing with clinical
assessment and treatment, and has been involved in both private practice
and institutional clinical work. He is currently serving as staff psychologist
on the schizophrenia ward at Solomon Carter Fuller Mental Health Center in
Boston, Massachusetts.
ROGER manvell, University Professor and Professor of Film at Boston
University, is well known as a biographer and author of many books con-
cerning film history and other cinema studies. Formerly director of the Brit-
ish Film Academy in London, he was also in 1973 Bingham Professor of
Humanities at the University of Louisville, Kentucky. Fie holds a Ph.D.
from London University, a D.Litt. in film studies from Sussex University in
England, as well as Honorary doctorates from Leicester University, En-
gland, and the University of Louisville. With Heinrich Fraenkel he has
written many biographical and other studies relating to the Third Reich, and
his more recent individual works include Films and the Second World War,
Shakespeare and the Film, Theatre and Film, and biographies of Hitler,
Charles Chaplin, and the actress, Ellen Terry. He was also Editor-in-Chief of
The International Encyclopedia of Film.
