Summary
This study contains analysis of several aspects of geomedical situ-
ation in the Baikal region. Under geomedical situation we mean an ag-
gregate of environmental factors, such as climate, ecology or epidemic
background, historically formed specific features of social development
(sociocultural characteristics, social institutions, economic models) and
complex relationships of a society and its social institutions, such as do-
mestic policy, state management, security issues, foreign policy), which
determines patterns of emergence and spread of human diseases and,
simultaneously, traditional and institutionalized health protection prac-
tices.
Our research team concentrated attention on three main research seg-
ments: 1) historical context of emergence of geomedical problems in the
Baikal region and their interconnections with geopolitical imperatives of
Russia / USSR in Inner and Central Asia in the period from the 1880s
throughout the first half of the 20th century; 2) perception of diseases in
the traditional nomadic society, the mainstay of the ethnocultural space of
the region, and on this basis, the making and fvmctioning of folk medicine
of the Mongolian peoples; 3) contemporary challenges and prospects of
development of regional medicine using a case study of the Republic
of Buryatia. The example of the Republic of Buryatia may be used as a
certain matrix of healthcare problems of the entire region as it aggregates
most characteristic problem factors of the entire Baikal region, such as
subsidized economy, inefficient state management and dispersed popula-
tion.
The in-depth study of these aspects of geomedical situation in the
Baikal region enabled us to draw several conclusions. First, as far as
history is concerned, the geomedical situation in the Baikal region was
shaped by a number of endogenous and exogenous factors. Among the
former were epidemic diseases common to humans and animals, such as
anthrax, bubonic plague, rabies and foot and mouth disease. There were
also endemic diseases, such as plague, whose epidemic focus lies within
the region’s borders (Transbaikalia and East Mongolia). One of the en-
dogenous geomedical factors was also prevalence of shamans and lamas
among the practitioners of folk and traditional medicine in the region that

affected the health of Buryats, Mongols and other indigenous peoples of
the region and partially explained low trust to European medicine in the
region in the period prior to the October revolution of 1917.
Second, the historical context of geomedical problems in the region
demonstrates an obvious interconnection with the aspects of impe-
rial Russian and Soviet geopolitics in East, Inner and Central Asia and
geomedical factors. For example, for a long time Buiyats were exempt
from the state healthcare policy in the region and basically left to their
own devices, such as shamans and Buddhist lama healers for an obvi-
ous geopolitical reason. The imperial government attached special im-
portance to a possible role of Buiyats and frontier guards in the contested
Russian-Qing borderland in Transbaikalia and, as a recognition of their
geopolitical significance granted them a rather broad self-administration.
Any interference of Russian state bureaucrats into the internal affairs of
Buryats was legally limited by the Statute of 1822. Since physicians were
obviously state employees and manifested imperial power where things
concerned health issues, they also were restricted to interfere. Therefore,
Buryats helped themselves with most healthcare issues up to the time the
Buryat ASSR was proclaimed in 1923.
The early Soviet years once again brought forth the geopolitical agen-
da. This time it was disguised as die Comintern policy in Asia. Alongside
with solving health problems, such as prevalence of syphilis in Buryat
population, the Bolsheviks invested Buryats with a peculiar geopoliti-
cal mission. They were an example of socialist transformations for the
Mongolian peoples of Inner Asia. At the same time, the Buryats were
supposed to facilitate export of Communist ideology in the Mongolian
and Buddhist world.
Geopolitical agenda and geomedical issue got entwined into a com-
plex knot when the USSR started to render medical and sanitary assist-
ance to the neighboring Asian states, such as Mongolia, Tuva, Afghani-
stan, Chinese province of Xingjian, Iran and so on. The Soviets used
medicine as a channel to transmit their ideology into mostly nomadic
traditional societies of Inner and Central Asia. At the same time, Soviet
medicine served as a “soft power” lever, creating strong competition with
other foreign medical missions in places like Urumqi in Xingjian and
gaining high esteem in the local population.
The traditional views of the Mongolian peoples on the origins and
nature of diseases reflected their relationships with the nature, spiritu-

al foundations of nomadic life, rational construction of life-sustaining
space around them. Shamanism played a key role in virtually all healing
practices. The origin of infectious, epidemic and mental diseases could
not be perceived by nomads beyond the supernatural realm. Therefore,
shamans, naturally invested with supernatural powers to communicate
with spirits, were the most authoritative healers in nomadic communi-
ties. Pragmatic Bolsheviks skillfully used this blind belief in miracles to
win over the population by using modem medicines to cure the seem-
ingly hopeless patients. To the Mongolian nomads such miraculous cure
became increasingly more convincing than mystic practices of shamans
and lama doctors.
However, mercilessly discarding everything associated with tra-
ditional healing practices, Soviet physicians refused to understand the
intricate social mechanisms behind various pathological processes in
Mongolian communities. At the same time they also irretrievably lost
precious empirical knowledge of healing properties of herbs, animal and
plant produce, minerals, which the nomads had accumulated over the
millennia of painstaking observation of fauna and flora.
Modem healthcare system of the Republic of Buryatia encounters
various challenges in the spheres of management, quality control, organi-
zation of an efficient and optimal network of medical institutions, preven-
tion of diseases and propaganda of healthy lifestyle. Its management is
bulky and bureaucratic. Development programs are mostly centralized.
Not a single hospital or outpatient hospital in Buryatia has a development
program specially tailored to its needs and problems.
Healthcare system of the Republic of Buryatia is currently in bad need
of a new model of quality control management and protection of patients’
rights. Based on the results of a sociological research, our sociology seg-
ment proposes creation of an inter-departmental medical expert commis-
sion that would be capable of coordination interaction of all stakeholders
in the process of improving the quality of medical services in Buryatia.
The analysis of the contemporary condition of healthcare shows that
the system in the region still rests on the Soviet-era foundations. It is a
complex milieu largely inept to reforming. Painstaking work is required
to bring it up to international standards by increasing its quality and ef-
ficiency.
