Applications of health information technology (health IT) are now wide-
spread in the form of electronic medical records (EMRs), greatly reshaping
the practice of clinical pediatrics. Population health stands to benefit

social and physical environment. Before this potential can be realized,
population health information models must be integrated into the design
and evolution of EMRs and other data sources.

Peer-to-peer benchmarking is an important component of rapid-cycle per-
formance improvement in patient safety and quality-improvement efforts.
Institutions should carefully examine critical success factors before
engagement in peer-to-peer benchmarking in order to maximize growth

a high-yield engagement for Children s Hospital of Pittsburgh of University

Initially described more than 50 years ago, electronic health records
(EHRs) are now becoming ubiquitous throughout pediatric health care set-
tings. The confluence of increased EHR implementation and the exponen-
tial growth of digital data within them, the development of clinical
informatics tools and techniques, and the growing workforce of experi-
enced EHR users presents new opportunities to use EHRs to augment
clinical discovery and improve pediatric patient care. This article reviews
the basic concepts surrounding EHR-enabled research and clinical

discovery, including the types and fidelity of EHR data elements, EHR data
validation/corroboration, and the steps involved in analytical interrogation.

Over the past 15 years, with alarming and illustrative reports released from
the Institute of Medicine, quality improvement and patient safety have
come to the forefront of medical care. This article reviews quality improve-
ment frameworks and methodology and the use of evidence-based guide-
lines for pediatric emergency medicine. Top performance measures in
pediatric emergency care are described, with examples of ongoing process
and quality improvement work in our pediatric emergency department.

Quality-based regulations, performance-based payouts, and open report-
ing have contributed to a growing focus on quality and safety metrics in
health care. Medical errors are a well-known catastrophe in the field. Espe-
cially disturbing are estimates of pediatric safety issues, which hold a
stronger capacity to cause serious issues than those found in adults.
This article presents information collected in the past 2 decades pertaining
to the issue of quality, and describes a preliminary list of potential solutions
and methods of implementation. The beginning stages of a reconstructive
journey of safety and quality in a Michigan pediatric hospital is introduced
and discussed.

In medicine, providers strive to produce quality outcomes and work to
continually improve those outcomes. Whether it is reducing cost,
decreasing length of stay, mitigating nosocomial infections, or improving
survival, there are a myriad of complex factors that contribute to each
outcome. One of the greatest challenges to outcome improvement is in
pediatric intensive care units, which tend to host the sickest, most com-
plex, smallest, and frailest of pediatric patients. This article highlights
some studies and advances in informatics that have influenced intensive
care unit outcomes.

The role of resident and fellow trainees in patient-centered improvement
processes is critical to a health care system s success. There is a growing
impetus to incorporate patient safety and quality improvement into the
educational framework of physicians in training. As part of the Next
Accreditation System, practice-based learning and improvement and
systems-based practice domains mandate that residents and fellows be
assessed on their ability to enhance the quality of care and advocate for

patient safety. Best practices for incorporating quality improvement and
patient safety into the curriculum of residents and fellows remains an
area of interest for educators.

Health care in the United States is plagued by errors, inconsistencies, and
inefficiencies. It is also extremely costly. Clinical pathways can drive high-
value care and high reliability within a health care organization. Clinical
pathways are much more than just guidelines or order sets as a part of a
protocol of care, however; they must incorporate multiple elements that
are critical to their successful implementation and sustainability. Addition-
ally, clinical pathways can be utilized to accomplish strategic goals of the
organization while fulfilling the quality, safety, and clinical aspects of the
organization s mission.

Patient safety and quality are 2 of many competing priorities facing health
care providers. As safety and quality rise on the agenda of executives,
payers, and consumers, competing priorities, such as financial sustainabil-
ity, patient engagement, regulatory standards, and governmental de-
mands, remain organizational priorities. Nursing represents the largest
health care profession in the United States and has the ability to influence
the culture of patient safety and quality. It is essential for hospital leader-
ship to provide a culture whereby nurses and staff are actively engaged
and feel comfortable speaking up. Transparency is critical in the strategy
and implementation of improving quality and safety.
Pediatric Quality Improvement: Practical and Scholarly Considerations 341

This article describes important aspects of health-care quality, quality
improvement (Ql), patient safety (PS), and approaches to research on
QI/PS efforts. Common terminology to facilitate an understanding of Ql
and PS research is reviewed. Models for understanding system and pro-
cess performance are discussed. Introductory considerations to Ql data
and Ql research analytical considerations are provided.

Emerging changes in the United States healthcare delivery model have
led to renewed interest in data-driven methods for managing quality of
care. Analytics (Data plus Information) plays a key role in predictive risk
assessment, clinical decision support, and various patient throughput
measures. This article reviews the application of a pediatric risk score,
which is integrated into our hospital s electronic medical record, and
provides an early warning sign for clinical deterioration. Dashboards that
are a part of disease management systems, are a vital tool in peer bench-
marking, and can help in reducing unnecessary variations in care.

This article examines the current role of telehealth as a tool in the delivery
of pediatric health care. It defines telemedicine and telehealth and pro-
vides an overview of different types of telehealth services. The article
then explores the potential of telehealth to improve pediatric health care
quality and safety through increased access to care, enhanced communi-
cation, expanded educational opportunities, and better resource utiliza-
tion. It also discusses current challenges to the implementation of
telehealth, including technological, financial, and licensing barriers, as
well as provider, patient, and legal concerns.
