The technological advances in the surgical correction of pelvic floor
defects are in large part the results of efforts to use minimally invasive
methods and techniques. Although many argue that vaginal surgery is
the original form of minimally invasive surgery, laparoscopic surgery
may offer advantages in several areas. In the cases of stress urinary
incontinence, and of anterior, apical, and posterior vaginal compart-
ment prolapse, laparoscopic approaches are available and may be
advantageous for surgeon and patient alike. Newer techniques and
technologies of vaginal surgery with mesh augmentation are burgeon-
ing and deserve careful consideration.
Total and Supracervical Hysterectomy 651

Hysterectomy is the most common non-pregnancy-related surgical
procedure among women. Patients who undergo laparoscopic hyster-
ectomy demonstrate more rapid recovery and decreased perioperative
morbidity compared to those who undergo laparotomy and may
experience less postoperative pain compared to the vaginal approach.
Recommendations for preoperative work-up and counseling are pro-
vided, including data regarding cervical retention versus removal.
Additionally, detailed procedural recommendations for laparoscopic
hysterectomy are outlined.

The discovery of an adnexal mass is a common clinical problem
affecting women of all ages. The primary goal of diagnostic and surgical
evaluation is the exclusion of malignancy. This review will detail recent
advances in diagnosis and treatment of adnexal masses. We will also
review the appropriate use of and advances in minimally invasive
surgical techniques that have the potential to decrease unnecessary
orbidity among patients during evaluation of adnexal masses.

There is good evidence that laparoscopic surgery is effective for the
treatment of endometriosis associated pelvic pain. However, it is
important that the diagnosis be accurate and that operative treatment
be optimal when surgical treatment is used for endometriosis. Common
pitfalls in surgical treatment and several aspects of surgical technique
are reviewed to help the gynecologic surgeon optimize treatment.

Science becomes art and art becomes function when fundamental
principles are utilized to dictate surgical practice. Most important, the
risk for inadvertent thermal injury during electrosurgery can be mini-
mized by a sound comprehension of the predictable behaviors of
electricity in living tissue. The aim of energy-assisted surgery is the
attainment of anatomic dissection and hemostasis with the least
amount of collateral damage and subsequent scar tissue formation.
The surgeon s final view of the operative field should accurately
approximate the topography discoverable after postoperative healing.
Uterine Leiomyomas, Current Concepts: Pathogenesis, Impact on
Reproductive Health, and Medical, Procedural, and Surgical Management 703

Uterine leiomyomas are highly common benign uterine tumors that
have a prevalence, by age 50, of about 70%. Often, leiomyomas are
asymptomatic, and disorders such as abnormal uterine bleeding and
infertility are likely related to lesions adjacent to or involving the
endometrial cavity. Interventions including medical management, myo-
mectomy, uterine artery embolization, hysterectomy, and other, inves-
tigative approaches are discussed. By the time they are diagnosed,
leiomyomas have frequently incurred irreparable harm; therefore, future
strategies should involve early diagnosis and the implementation of
medical or targeted procedural intervention with minimal morbidity and
cost that at the same time preserve uterine function.
Simulation and Education in Gynecologic Surgery 733

Simulation training is becoming a valuable part of surgical education
resulting in an improved level of acquired surgical skills which can be
transferred into the operating room. This is occurring at a time when
there is an increasing demand for improved education in healthcare,
and a growing impatience with the high error rate. Because both
training and practicing surgeons are increasingly being held account-
able for their error rates, simulation will continue to play an important
role in the training and evaluation of gynecologic surgeons.

Laparoendoscopic Single-Site Surgery in Gynecology 741

Although not a new concept, recent advances in surgical instrumenta-
tion have resulted in a renewed interest in the single-puncture approach
to a variety of gynecologic procedures. These advances include novel
designs for port systems, articulating instrumentation, and flexible
laparoscopes. Herein, we describe strategies for overcoming the tra-
ditional obstacles associated with utilizing multiple instruments via a
single incision to triangulate to a target. These obstacles include
external instrument clashing and internal sword fighting. Evidence
that supports a potential benefit to this approach is also described.
Laparoscopy in Pregnancy and the Pediatric Patient 757

Gynecologic laparoscopy has gained momentum in the past 2 de-
cades. It is considered the standard of care in the treatment of certain
pathology in the pediatric and adolescent populations such as endo-
metriosis, ovarian cystectomy, ovarian torsion, and ovarian transpos-
tion. The application of laparoscopy in pregnancy, however, has been
slow to gain widespread acceptance. Physiologic and anatomic
changes in the pregnant patient introduce certain risks unique to this
population. Although pregnancy is considered a relative contraindica-
tion for laparoscopic procedures, data indicate that these concerns
may be unwarranted when procedures are performed by skilled sur-
geons. Appendicitis, cholecystitis, ovarian torsion, and symptomatic
adnexal masses are some of the more common conditions necessitat-
ing intervention. Although prospective studies of laparoscopic surgery
in pregnancy are lacking, there are numerous case reports affirming the
feasibility of this approach.
The Core of a Competent Surgeon: A Working Knowledge of Surgical
Anatomy and Safe Dissection Techniques 777

The competent gynecologic surgeon has a sure, working knowledge of
the anatomy in the field of pelvic dissection and is expert in the
techniques and in the millimeter by millimeter progression of surgical
dissections. When operating in the pelvis, several questions arise: In
what anatomic area am I dissecting? defines the anatomy to be
dissected out. What dissection techniques will I use here? gives the
surgeon the confidence to proceed with the operation, while safeguard-
ing the integrity of the surrounding anatomic structures. With less blood
loss and less trauma, the surgeon may expect a better patient outcome.
