The concept of female beauty has changed throughout time, but the form and size of the
breasts and gluteal region have remained constant as symbols of maximum femininity.
Sculptures and prints show us feminine figures that are voluminous and reflect human
history s interest in fertility. The early years of gluteal augmentation saw few published
reports that described the procedure technique, follow up, or possible complications.
But developments continued as surgeons began experimenting with different
anatomical planes for implant placement. The most important goal in plastic surgery
is meeting a patient s expectations. It is important for the surgeon to thoroughly explain
to patients what can realistically be achieved with a procedure.

The authors describe four characteristics that determine attractive buttocks in addition
to the balanced anatomical dimensions of the maximal point of gluteal projection.
Based on this information, defects of the gluteal region can be classified into five types.
The most appropriate size and shape of gluteal implants and the proper surgical
technique for gluteal augmentation can be chosen by first identifying the buttock type of
a patient. The proposed method of buttock classification helps achieve a gluteal contour
that closely matches the standard of beauty.

increased patient demand for buttock reshaping and augmentation. This phenomenon
is reflected in statistics collected by the American Society for Aesthetic Plastic Surgery,

Increasing patient demand has necessitated a more structured approach to evaluation of
gluteal anatomy and the development of surgical procedures to enhance its beauty. The
proposed classification system for gluteal contouring focuses on evaluating and
identifying the different frame types, the different gluteal muscle types, and the
relationship between the muscle and the frame. Finally, a ptosis classification system is
presented.

can improve the cosmetic results of gluteal augmentation and thereby enhance patient

Universal Allure of the Hourglass Figure: An Evolutionary Theory of Female

So what explains the universal and enduring appeal of the hourglass figure? One
explanation based on evolutionary psychological theory is that female beauty as
represented by the hourglass figure taps into important biological information about
various factors regulating women s reproductive potential and fertility. This article
briefly describes basics tenets of evolutionary psychology pertaining to the nature and
significance of female attractiveness. The article also summarizes experimental and
clinical evidence demonstrating a link between the hourglass figure and hormonal
and endocrinological mechanisms regulating reproductive potential, fertility, and risk
for major diseases. Such evidence is crucial to support the claim that attractiveness of the
female figure is a reliable cue to a female s reproductive capability and good health. Also
presented in this article is evidence that changes in the hourglass figure alone
systematically affect female attractiveness judgments of lay and professional men and
women not only in our society but in various and diverse societies. Finally, evidence is
presented showing that equating beauty with the hourglass figure is not a novel or recent
phenomenon shaped by the mass media; allure of the hourglass figure is evident across
generations in ancient cultures.
Universal and Ethnic Ideals of Beautiful Buttocks are Best Obtained
by Autologous Micro Fat Grafting and Liposuction 371

Recently there has been a dramatic increase in the number of patients seeking buttocks
enhancement and in the degree of augmentation requested. To fulfill these requests,
aesthetic plastic surgeons must understand the patient s personal requests and ethnic
identity, as well as any universal ideal of proportions and contours that create the
impression of beautiful buttocks. Universally perceived ideal buttocks are 1.4 times the
circumference of the waist, which is consistent cross culturally and throughout history.
Beyond this are important ethnic differences in the image of perfect buttocks shape. The
combination of autologous micro fat grafting and liposuction is the best and possibly
only way to obtain various ideal shapes, and offers a lower incidence of complications
compared with buttock implants.
Gluteal Recontouring With Combination Treatments: Implants, Liposuction,and Flat Transfer 395

Patients seeking augmentation or recontouring of the gluteal region have three basic
options: gluteal implants, liposuction, and augmentation with fat injections. In many
cases, all three treatments are used together to achieve attractively shaped buttocks with
proper proportions and good projection. The treatment or combination of approaches
that is appropriate for each individual patient is discovered through consultation that
focuses on understanding a patient s goals. To determine the treatment(s) required by
a patient, his or her unique anatomy must be analyzed to identify gluteal areas that have
excesses or deficiencies that can be successfully addressed in gluteal recontouring.

academic adventure. During these years my coworkers and I have progressively
improved surgical technique and devised an anatomical system for gluteal augmenta¬
tion that includes an ideal implant design and templates to assist in evaluating patients
in the preoperative period and to identify the most appropriate implant size.

results and, when properly performed, has a low incidence of complications. Using the
intramuscular tissue plane ensures that implants are naturally positioned. In addition,
larger implants may be used than with the submuscular technique, which many patients

provides the greatest amount of implant coverage and pads the implant with muscle

Over the past several years, there has been a tremendous growth and interest in buttocks
augmentation in the United States. Surgical techniques have evolved over time to correct
anatomical deficiencies and fulfill patient requests, including silicone implant
placement (subcutaneous, intramuscular, submuscular, and subfascial) and autologous
micro fat grafting. Unfortunately, these techniques have presented great challenges with
regard to the incidence, diagnosis, management, and prevention of various postsurgical
complications. Extensive collaboration and transparency in discussing complications
has resulted in refinement of both surgical technique and medical management,
resulting in improved outcomes for patients undergoing buttocks augmentation.
Buttocks Lifting: How and When To Use Medial, Lateral, Lower, and Upper Lifting

New buttocks lifting techniques are evolving rapidly, with some so recent that very few
surgeons have performed them. However, a few new principles may be drawn from the
various techniques in an attempt to produce better results and minimize scars. The
indications for use of a medial, lateral, lower, or upper buttocks lift are presented in this
article. There is no technique for all seasons and a surgeon who intends to perform
a buttocks lift must evaluate the four techniques and the indications for each. An
intramuscular implant helps to improve results even further and should be used when
appropriate.

Massive weight loss (MWL) patients have unique and severe gluteal deformities that
demand alternative body contouring solutions because they are rarely candidates for
gluteal implants. Techniques for autologous gluteal augmentation (AGA) with
circumferential body lift (CBL) are offered as a solution to this problem. Experience
with these techniques will allow surgeons to extend AGA to other aesthetic patients
presenting with flattened buttocks. Flap indications, surgical planning, techniques,
results, complications, and postoperative management are discussed.
