Genetic causes of human infertility are diagnosed infrequently in
clinical practice. Most single gene disorders are rare and produce
a phenotype of absent or impaired pubertal development and in¬
fertility. Most patients with infertility, however, have no manifesta¬
tions of pubertal delay. Physicians should be aware of some of the
most common genetic causes of infertility, such as cystic fibrosis,
fragile X syndrome, Y chromosome deletions, and chromosome
disorders because they have clinical implications. The molecular
basis for most types of infertility remains unknown, but with the
completion of the Human Genome Mapping Project, new advances
are continually being made.

Central Causes of Hypogonadism—Functional and Organic 593

Although great strides have been made to elucidate the numerous
origins of hypogonadotropic hypogonadism, many of the causes
of this complex disorder remain elusive and under continued
investigation. In the past few years, gonadotropin releasing
hormone (GnRH) has emerged as an important feature of the
hypothalamic pituitary gonadal axis, and its role in mediating
the reproductive axis must be duly noted. Among the central
causes of hypogonadism, hypothalamic amenorrhea, caused by
suppression of the GnRH pulse generator, counts as one of the
more significant issues for women. This article discusses hypotha¬
lamic amenorrhea and its subclassifications—for example, exercise
and anorexia nervosa, among other factors. Other topics covered
include the roles of lesions (eg, craniopharyngiomas), exogenous
toxins (eg, anabolic steroids), and classic syndromes (eg, Bardet
Biedl, Prader Willi, and Kallmann s syndromes) in the develop¬
ment of hypogonadotropic hypogonadism.
Mechanisms of Follicular Dysfunction in 46,XX Spontaneous
Premature Ovarian Failure 613

Previously, premature menopause was the term used to describe
46,XX spontaneous premature ovarian failure. For young women
desiring pregnancy, this term implies absolute and irreversible in¬
fertility—finality caused by follicular depletion. This is clearly not
the case. Pregnancy can occur after this diagnosis. Insights into
the pathophysiology of this disorder make it clear that approxi¬
mately one half of young women who have 46,XX spontaneous
premature ovarian failure have follicles remaining in the ovary. On¬
going research is uncovering the mechanisms of follicular dysfunc¬
tion in these women.
Assessment and Management of Anovulatory Infertility in
Polycystic Ovary Syndrome 639

Polycystic ovary syndrome is the major cause of anovulatory infer¬
tility. Fertility can usually be restored by appropriate choice of
induction of ovulation, but careful monitoring is required, even
when using clomiphene.

This article presents a summary of the disease process and reviews
the literature on the diagnosis and treatment of endometriosis
associated infertility. The purpose of the article is to provide prac¬

translate into improved outcomes for women suffering from this
disorder.

A progressive decline in fecundity with advancing age is a reality
attributed primarily to the detrimental impact of various processes
on female gametes. Declining numbers and deteriorating quality of
oocytes contribute to reduced clinical pregnancy rates and escalate
pregnancy wastage in aging women. The age of the male partner
does not seem to have a major impact on the eventual fertility of
the aging couple.
Male Factor Infertility 689

Half of infertile couples have a component of male factor infertility.
Only after the male partner has been completely evaluated can
these couples be offered the most productive and cost effective
treatment options. This article provides a brief review of the eval¬
uation of infertile men and common pathologic conditions leading
to male infertility.
Evaluating the Efficacy of Intervention 709

Conditions associated with infertility include age, endocrine, ge¬
netic, and anatomic factors. A variety of treatments have been
proposed for these conditions, as well as for infertility. The funda¬
mental problem in assessing the efficacy of interventions for infer¬
tility is that, with few exceptions, there will be some baseline
probability of pregnancy independent of the intervention. Thus,
the goal of infertility study design and data analysis is to determine
the incremental success rates that can be attributed to treatment
above this baseline level. This article evaluates the standards of
intervention efficacy and discusses how physicians should assess
whether these interventions may be successful.
