The approach to hazard identification and exposure characterization presented in this
article will assist the physician in completing an evaluation of a patient s workplace and
home exposures. The approach encourages physicians to gather basic information about
their patients environments in an effort to characterize their exposure to agents that may
contribute to lung disease.
Acute Inhalation Injury 707

Recent events have underscored the importance of proper diagnosis and management of
patients with inhalation injury. Clinicians who care for individuals who have sustained
inhalation damage to their respiratory tract need to take a careful exposure history and
be alert to possibilities of delayed effects and clinical deterioration. Although supportive
care and prevention remain the cornerstone of current approaches to this condition, a
better understanding of the mechanisms of cellular injury and repair may lead to
improved treatments in the future.
Occupational Upper Airway Disease 717

The upper respiratory tract, which extends from the nares to the larynx, performs several
physiological functions including olfaction, filtering, phonation, air conditioning, and
microbial defense. Many of these functions point to the primary role that the upper air¬
way plays in defending the lungs from potentially noxious exposures. In the occupa¬
tional setting there are many chemical and biological agents that can induce allergic or
irritant effects on the upper airway. In addition to allergic and irritant forms of rhinitis,
irritant associated vocal cord dysfunction has been increasingly recognized as an occu
pationally related disorder. Irritant associated vocal cord dysfunction is an important
differential diagnosis for reactive airways dysfunction syndrome, particularly when
there is a poor response to typical asthma therapy. Understanding the spectrum of
pathophysiological responses of the nose, sinuses, and larynx to various workplace
agents is essential in the formulation of a rational approach to the clinical assessment
and management of occupationally related upper airway disorders.

Short term, high level exposures to dusts, gases, mists, fumes, and smoke that are irri¬
tating to the respiratory tract are capable of inducing asthma, the so called reactive
airways dysfunction syndrome. Such exposures, however, do not occur frequently;
chronic or recurrent exposures to lower levels of irritants are much more common. This
article reviews the evidence that supports the concept that low level exposures to respi¬
ratory tract irritants can contribute to the development of chronic obstructive pulmonary
disease and asthma.
New Developments in Work Related Asthma 737

Work related asthma (WRA) is the most commonly recognized occupational lung dis¬
ease, and remains underrecognized and underreported. New onset occupational asthma
(OA) and work aggravated asthma (WAA) can have deleterious medical and socioeco
nomic consequences for the individual. This article focuses on recent developments in
the understanding of the prevalence of WRA, the results from ongoing longitudinal
inception cohort studies, and the application of new methods of studying airway inflam¬
mation in workers at risk of developing WRA. Although interpretation and comparison
between studies is difficult, between 10% and 20% of new or aggravated adult asthma
has important work related factors.
Environmental Tobacco Smoke and Adult Asthma 749

Environmental tobacco smoke exposure (ETS) is common in the general US population
and among adults with asthma. The epidemiologic evidence indicates that ETS exposure
is a risk factor for new onset adult asthma. Among adults with established disease, ETS
exposure seems to be a cause of asthma exacerbation. These findings should provide fur¬
ther support for policies aimed at creating smoke free environments.
Update on the Health Effects of Outdoor Air Pollution 763

The number of studies conducted on the health effects of air pollution has increased
exponentially. Important methodological advances include the application of novel
observational study designs, in particular the multi city design, and the development
and application of airborne particle concentrators for use in experimental human expo¬
sure studies and toxicological studies. Experimental data are validating and providing
insight into some surprising observational findings.
Pulmonary Manifestations of Intentionally Released Chemical and Biological Agents 777

Recent events have underscored the importance of knowledge and understanding of
biological and chemical agents that have been intentionally released on civilian popula¬
tions. Preparedness of the medical community to recognize and manage the resulting
clinical syndromes will be a major determinant in the outcome of such an attack. Patients
or clusters of patients who present with febrile syndromes that are unusual for the geo¬
graphic or seasonal setting should trigger notification of public health authorities and
use of state or national reference lab systems for augmented diagnostic support. In many
cases, early, empiric therapy, before definitive diagnosis, is required for survival.

Agricultural work is associated with high rates of injury, disability, and illness. Agri¬
cultural workers are at increased risk for a variety of illnesses including respiratory
disorders, dermatologic conditions, and cancer. The recognition of organic dust toxic
syndrome (ODTS) has led to increased understanding of acute illness in farmers and
grain workers. Previously, many cases of acute illness were probably erroneously called
farmer s lung. It has also become apparent that the same agents that are responsible for
ODTS are responsible for the high prevalence of bronchitis in certain agricultural work¬
ers. The recent description of the innate immune system is very exciting because it will
lead to increased understanding of the pathogenesis of organic dust induced disorders.
Update on Respiratory Disease from Coal Mine and Silica Dust 811

Excessive exposure to coal, coal mine, and silica dust causes a variety of pathological
responses in susceptible hosts, including pulmonary fibrosis or pneumoconiosis and
chronic obstructive pulmonary disease and their resultant restrictive, obstructive, or
mixed pattern pulmonary function impairments. For many years, much of the discussion
on the respiratory health effects of exposure to these dusts was restricted to the fibrosing
pulmonary tissue reactions in response to retained dust. It excluded other reactions of the
pulmonary parenchyma and airways to the dust, which are extremely important, and can
result in significant impairment in sensitive individuals. This article discusses the broad
spectrum of pulmonary toxicity to these inorganic dusts, clinical evaluation, and man¬
agement of patients with respiratory disease from dust exposure, as well as the associa¬
tion between silica exposure and an increased risk of pulmonary malignancy.
Genetic and Exposure Risks for Chronic Beryllium Disease 827

Exposure to beryllium results in beryllium sensitization and the granulomatous lung
disease in 2% to 19% of those exposed. This low frequency of disease, findings of par¬
ent child and sibling pairs with disease, and animal models of disease restricted by
genetic differences indicate a genetic susceptibility for sensitization and disease. This
article reviews our current understanding of the clinical presentation and evaluation of
beryllium related health effects, the immunopathogenesis of these processes, and the
exposure and genetic factors associated with increased risk of developing sensitization,
chronic beryllium disease, and severe forms of the disease. Exposure and genetic sus¬
ceptibility factors likely act in concert in the risk of sensitization and disease.
Pulmonary Impairment and Disability 841

This article reviews the evaluation of impairment and disability caused by respiratory
diseases, including common occupational and environmental respiratory diseases. It is
intended to provide a framework by which practicing physicians may understand their
role and responsibilities in the determination of disability. The respiratory system dif¬
fers from many other organ systems; objective measures of pulmonary function exist
and are readily applicable to the evaluation.
