Chronic Hypertension in Pregnancy 447

Hypertension is the most common medical disorder during preg¬
nancy. Chronic hypertension is a serious medical complication
in pregnancy associated with increased maternal and perinatal
morbidity and mortality. Maternal complications include abruptio
placentae, stroke, and superimposed pre eclampsia. Fetal compli¬
cations include prematurity, stillbirth, low birth weight, and neo¬
natal death. Careful antepartum, intrapartum, and postpartum
management of women with chronic hypertension in pregnancy
may reduce morbidity and mortality.

Few specialties in medicine have witnessed such dramatic
changes in the twentieth century as cardiology and cardiovascular
surgery. Women of reproductive age and pregnant women have
undoubtedly benefited from these advances. This article discusses
the implications of cardiac disease and its therapies for each stage
of pregnancy, including delivery, and touches on postpartum
considerations.

Women with diabetes mellitus that precedes pregnancy have pre¬
dictable but modifiable risks of fetal malformation, fetal mac

rosomia, fetal demise, and maternal pyelonephritis, hypoglyce
mia, hyperglycemia, and hypertension. Women diagnosed with
gestational diabetes very early in pregnancy most likely had
pre existing disease and have outcomes similar to pregestational
diabetics. Most cases of gestational diabetes are diagnosed with
glucose tolerance testing. Despite the large resource allocation to
the detection and monitoring of glucose intolerance in pregnancy,
attributable benefits are few and difficult to quantitate.
Asthma in Pregnancy 537

Contemporary management of the pregnant asthmatic is directed
at controlling symptoms and treating the underlying cause of
asthma, namely, inflammation. This article discusses the patho
physiology of asthma and the effects of asthma on pregnancy and
vice versa and reviews the National Asthma Education Program
guidelines for the treatment of asthma in pregnancy.
Pneumonia In Pregnancy 553

Cardiopulmonary adaptations of pregnancy reduce respiratory
and cardiac reserve and, in the setting of pneumonia, can lead to
maternal decompensation. Such decompensation can culminate
in fetal compromise, preterm labor, and delivery. This article
reviews the incidence, etiology, pathophysiology, diagnosis, treat¬
ment, and complications related to pneumonia in pregnancy.
Disease of the Gallbladder and Pancreas in Pregnancy 571

Disease affecting the gallbladder and pancreas can deeply impact
fetal outcome. The pathophysiology and management of gallblad¬
der and pancreatic disease in pregnancy is discussed in detail.
Prompt recognition and management markedly improves out¬

Urinary Tract Infections During Pregnancy 581

Urinary tract infections, which occur in 2% to 7% of all pregnant
women, may result in significant maternal and fetal morbidity.
Fortunately, screening for, and treatment of, asymptomatic bacte
riuria during pregnancy can prevent much of this morbidity.
Women at increased risk for urinary tract infections during preg¬
nancy include those with a prior history of urinary tract infection
during pregnancy, diabetes, certain hemoglobinopathies, and re¬
nal anomalies.

Women with renal disease who conceive and continue a preg¬
nancy are at significant risk for adverse maternal and fetal out¬
comes. Risk is inversely related to the degree of renal insuffi¬
ciency. Pregnancy induced changes in the urinary tract can
temporarily increase renal function compromise, such as nephro
sis, but most often results in no net increase in dysfunction.
Common complications of pregnancy—such as hypertension and
hypovolemia—can be associated with acute renal injury or aggra¬
vation of pre existing disease.
Seizure Disorders in Pregnancy 601

Epilepsy is the presence of two or more seizures in the absence
of an identifiable cause for the seizures; that is, no intracranial or
metabolic abnormality. Epilepsy affects approximately 1% of the
US population, which represents an estimated 1.1 million women
of reproductive age. The management of women with epilepsy
during pregnancy is the focus of this article.
Venous Thromboembolism and Pregnancy 613

Venous thromboembolism (VTE) is a frequently encountered com¬
plication of pregnancy. The signs and symptoms of VTE are
neither sensitive nor specific, and most occur in patients without
recognizable risk factors, making the diagnosis difficult. Given
the significant morbidity associated with the sequelae of VTE,
namely pulmonary embolism, prompt diagnosis and appropriate
treatment are of paramount importance.
