Editors Notes 1
Francine Cournos, Marshall Forstein
1. Essential Medical Facts for Mental Health Practitioners 3
Milton L. Wainberg, Marshall Forstein, Alan Berkman, Francine Cournos
Research is rapidly generating new information, which is leading to
new insight on the nature of the HIV virus and its epidemiology. In
addition, dramatic developments in effective antiretroviral medications
offer far greater treatment options.
2. Psychosocial Issues in Antiretroviral Treatment 17
Marshall Forstein
Placing the exciting new treatments in historical context helps the men¬
tal health provider see where we have been and where we are going.
Although treatments offer great hope, they are complex, as are the myr¬
iad psychosocial issues attendant to them.
3. Diagnosis and Treatment of HIV Associated 25
Neurocognitive Disorders
Stephen]. Ferrando
Highly active antiretroviral therapy has reduced mortality, but many of
these medications do not effectively cross the blood brain barrier. This
has led to a growing interest in the role of the central nervous system
in HIV replication and HIV related morbidity.
4. Psychotherapeutic Strategies for Coping with HIV Infection 37
Claire Zilber
Psychotherapy in a variety of forms can aid those coping with HIV and
AIDS to adapt and often to grow psychologically, socially, and spiritually.
5. Some Further Thoughts on Adjustment,
Coping, and Bereavement 45
Dottie Ward Wimmer, Jennifer M. Hackler
Now that HIV AIDS has become, for many, a chronic illness rather than
a virtual death sentence, individuals face far different yet equally com¬
plex psychosocial issues in coping with the challenges of their condition.
6. Mood and Anxiety Disorders 51
J. Stephen McDaniel, Andrew C. Blalock
Frequently, depression and anxiety are seen in HIV and AIDS patients
throughout the course of the disease. Effective symptom relief not only
improves quality of life but also may be linked to treatment adherence,
reduction in risk behavior, and decreased health care utilization.
7. Substance Abuse and HIV Infection 57
Stephen J. Ferrando, Steven L. Batki
Individuals with both psychiatric and substance use disorders may be
at increased risk for HIV, and substance use disorders often complicate
the treatment of HIV positive patients. These and other factors have led
to the development of integrated HIV, drug abuse, and psychiatric treat¬
ment services.
8. Severe Mental Illness and HIV AIDS 69
Karen McKinnon, Jennifer Rosner
Research demonstrates that persons with severe mental illness are at
particularly high risk for contracting HIV AIDS. Mental health care
providers need to assess their patients particular needs, educate them,
and make available to them the appropriate services from the wide
range that are available.
9. Preventing HIV Infection Through Sexual Behavior Change 77
Michael P. Carey
A wide range of sexual behavior change programs—targeted at indi¬
viduals, small groups, schools, communities, and public policy—have
been empirically tested and determined to be effective.
10. Adherence to Treatment 85
Milton L. Wainberg, Francine Cournos
For an increasing number of HIV positive patients, highly active anti
retroviral therapy is the standard of care, but to be successful, it
requires 95 percent adherence to the regimen. Suboptimal adherence
poses a health danger as it can lead to the development of multidrug
resistant strains of HIV.
11. Legal and Ethical Issues Relevant to HIV Positive 95
Psychiatric Patients
James Satriano
Public health laws are undergoing changes vis a vis HIV and AIDS. Key
issues include informed consent for HIV testing, the relevance of HIV
status in assessing dangerousness, and the obligation to warn poten¬
tially at risk individuals regarding HIV positive psychiatric patients.
12. Provider Issues in HIV Care 101
Larry K. Brown, Celia M. Lescano
The types of stress experienced by those caring for HIV and AIDS patients
have changed as new treatments have led to improved outcomes. Still,
providers can experience burnout and exhaustion as the issues they con¬
front are complex and emotionally taxing. Seeking the appropriate inter¬
ventions is just as important for the provider as for the patient.
13. An Integrated Program for Comprehensive HIV Care 111
Joyce Seiko Kobayashi, Walter L. Standridge
Disenfranchised populations have less access to the new antiretroviral
therapy. To reach these populations more effectively and maximize
adherence, HIV AIDS clinics should consider offering integrated psy¬
chiatric, substance use, and medical services. The Denver Health Med¬
ical Center offers one such program.
14. A Program for Families and Children Affected by HIV 119
Barbara Draimin
The Family Center is a program in New York City that has three
constituencies—parents with AIDS, their children, and the children s
potential new caregivers. The goals are for all of those closely affected
by the illness to learn to cope with their situation optimally while the
parent is alive and to ensure care for the children once the parent has
died.
15. A Practitioner s View from the Front Lines 125
Robert Stasko
Why would anyone want to see HIV positive patients? One practitioner
who is asked this question all the time gives his very personal answer.
