This article summarizes the highlights of research on the pathogenesis of
malignant tumors affecting the eyelids. Clarification of the genetic, biochem¬
ical, and immunological regulatory pathways allowing development and
progression of eyelid malignancies will lead to more effective and specific
therapy aimed at various levels of carcinogenesis.
Periorbital Reconstruction Using Novel Alloplastic Materials 571

In eyelid reconstruction, the surgeon is challenged to produce a functionally
and cosmetically acceptable result through the selection of appropriate re¬
constructive materials and techniques. Myriad techniques use autogenous
tissue to reconstruct defects of the upper and lower eylid, canthal tendons,
and lacrimal drainage system. At times, alloplastic materials are a necessary
adjunct to the use of autogenous tissue techniques. Additionally, alloplasts
are sometimes used instead of available autogenous tissue because of de¬
creased surgical time, absence of donor site morbidity, and the potential
for improved surgical results associated with their use. This article reviews
the use of several alloplastic materials in periorbital reconstruction.
Anophthalmic Orbital Implants 587

This article looks at a brief history of orbital implants followed by a discus¬
sion of modern day implants and current concepts. The various porous
orbital implants, pegging, and complications are discussed.

Orbital vascular lesions include a heterogeneous group of disorders that
have in common one or more of the cellular components of blood vessels.
Conflicting definitions of some lesions have complicated classification and
blurred management recommendations. For these lesions, the authors stress
a functional classification and present the clinical, imaging, and histologic
features of the major vascular lesions, and approach to their management.
Preseptal and Orbital Cellulitis 633

The bacteriology and treatment of preseptal and orbital cellulitis have
evolved since the introduction of the Hemophilus influenzae vaccine. Some
patients can be successfully treated as outpatients. Subperiosteal abscesses
in children often can be managed with intravenous antibiotics alone. Cos
metically acceptable surgical approaches have advanced orbital surgery and
treatment of orbital cellulitis.
Fungal Disease of the Orbit 643

Fungal infections of the orbit are uncommon but when present can be vision
and life threatening. The two most common fungal diseases with orbital
manifestations are mucormycosis and aspergillosis. Most of these infections
start in a paranasal sinus and secondarily invade the orbit. Mucromycosis
usually affects debilitated individuals, especially diabetics in ketacidopsis
or patients immunosuppressed after cancer chemotherapy. Aspergillosis
has several different clinical presentations and can affect debilitated and
normal individuals. Patients with advanced AIDS are also at risk of develop¬
ing sino orbital aspergillosis. This article reviews the clinical presentations,
imaging characteristics, diagnosis, treatment, and prognosis of patients with
fungal orbital infections. Newer therapies, including new antifungal medica¬
tions and hyperbaric oxygen, are discussed.
Management of Orbital Lymphoid Lesions 657

Lymphoid lesions comprise some of the most commonly occurring primary
tumors of the orbit. The REAL classification has been recently established
as a useful system for the categorization of lymphoid lesions, including
those arising in the orbit. The appropriate evaluation and management of
patients with orbital lymphoid lesions are discussed.
Recent Developments in the Evaluation and Treatment of
Lacrimal Gland Tumors 663

Clinical characteristics, diagnostic and pathologic features, and biological
behavior, and management protocols for many of the common lacrimal
gland disorders are reviewed. Because several of the lacrimal gland fossa
lesions are life threatening, thoughtful evaluation based on clinical history,
physical examination, and radiographic soft tissue contour analysis is of
paramount importance for proper differentiation. A new treatment ap¬
proach using intracarotid chemotherapy as an adjuvant therapy to the con¬
ventional radical orbitectomy and radiation for adenipoid cystic carcinoma
is presented. The rationale for intracarotid chemotherapy before exentera
tion and the treatment protocol are detailed. The sanguine survival outcome
of this new treatment modality for a devastating disease, in which successful
therapy remains elusive, warrants further studies.

Medical Treatment of Graves Ophthalmopathy 683

Graves ophthalmopathy is an autoimmune condition involving the orbital
tissues. Most patients with this condition require only supportive care and
reassurance; symptoms subside with time. Approximately 20% of patients
with more severe thyroid associated eye disease require immune modula¬
tion or surgical therapies. Systemic corticosteroid is the first line therapy.
Radiotherapy, cyclosporine, intravenous immunoglobulin, octreotide, and
plasmapheresis are other options, but in severe cases, combination therapies
may be needed.
The Surgical Treatment of Graves Orbitopathy: A Decade
of Progress 693

Surgical rehabilitative procedures for Graves orbitopathy include orbital
decompression, strabismus surgery, eyelid malposition correction, and
blepharoplasty. During the past decade, the surgical indications and tech¬
niques have evolved considerably. The authors review the latest concepts
in the surgical management of this challenging disease.
Congenital Nasolacrimal Obstruction 705

Congenital nasolacrimal obstruction is a common disorder with a variety
of causes and treatment options. This article reviews the embryology and
anatomy of the lacrimal outflow system and the possible etiologies of ob¬
struction. Medical and surgical treatment options are discussed, including
a review of the literature regarding controversies in managing these patients.
Acquired Nasolacrimal Duct Obstruction 719

Acquired nasolacrimal duct obstruction is most commonly caused by a
primary, idiopathic, fibro inflammatory process that results in occlusion of
the distal nasolacrimal duct. Secondary causes of lacrimal obstruction caused
by systemic disease, neoplasia, or adjacent sinus disease should be excluded.
A complete obstruction of the nasolacrinal duct may be determined by
lacrimal irrigation and canalicular probe; however, partial obstructions of
the nasolacrinal duct are more difficult to assess and may require a battery
of tests to localize the site of lacrimal drainage system obstruction. Surgical
treatment of acquired nasolacrimal duct obstruction is the rule, and medical
therapy has only a limited role. Although external dacryocytorhinostomy
remains the gold standard for establishing lacrimal drainage outflow, im¬
proving results of the endonasal dacryocytorhinostomy and of balloon da
cryoplasty support these techniques as viable alternative treatments of pri¬
mary nasolacrimal duct obstruction.
New Techniques in Eyebrow Surgery 731

The most significant advance in brow and forehead elevation surgery in
recent years has been the introduction of the endoscopic surgical technique.
Through small incisions in the hair bearing scalp, the endoscope is used to
visualize dissection planes, nerves, and arteries under high magnification.
With posterior mobilization but not excision of the scalp, the eyebrows and

forehead are raised to a new position with little risk of sensory loss and
alopecia. With greater patient acceptance than the open coronal brow lift,
the endoscopic approach is ideal for younger individuals with brow ptosis
and redundant upper lid tissue who need stabilization of the brows before
blepharoplasty.
Cosmetic Eyelid Surgery 749
Samuel A. Gallo, Ralph E. Wesley, Kimberly A. Klippenstein,
and Brian S. Biesman
Patients present to cosmetic eyelid surgeons with many concerns about the
appearance of the periorbital region. A treatment plan can be formulated
after performing a careful preoperative evaluation and discussing the rele¬
vant findings with the patient. Rejuvenating the periorbital area may require
surgery on the skin, eyelids, eyebrow, forehead, cheek, or the midface. Soft
tissue augmentation may be required to correct soft tissue deficiencies,
botulinum A injections may be needed to control hyperkinetic facial muscles,
and facial implants may be needed to support hypoplastic bony features.
Although the general ophthalmologist does not need to master all of these
procedures, a working knowledge is required of any surgeon who performs
cosmetic eyelid surgery.
Combined Oculoplastic and Neurosurgical Approaches to
Orbital Disease 765

Team approaches combining oculoplastic surgeons, neurosurgeons, ear nose
and throat, and craniofacial surgeons are being used increasingly to treat
orbital and anterior cranial fossa disease. The author describes several areas
in which combined surgical techniques have clearly made a difference in
approach to the treatment of the diseases.
