New Techniques in MR Imaging of Brain Tumors 691

In the past decade, an array of new MR imaging techniques have been
applied to brain tumors. With superior contrast and resolution, and with¬
out the need for image co registration, functional MR imaging techniques
may displace positron emission tomography (PET) as the pre eminent
modality for studying brain and tumor physiology and chemistry for
indications other than receptor based imaging. Nevertheless, the new MR
imaging techniques require further histologic, physiologic, and intraopera
tive validation in suitable animal models and in clinical studies, and
should be used to complement PET. Application of echo planar imaging
and other fast imaging sequences can merge several MR studies (e.g.,
perfusion imaging, diffusion weighted imaging, and MR spectroscopy)
into a 1 hour clinical examination. Synergistic information provided by
these new techniques might soon enable physicians to reach the ultimate
goals of noninvasive tumor grading and the avoidance of biopsy.
MR Imaging: Its Current and Potential Utility in the Diagnosis
and Management of Breast Cancer 715

Diagnostic and treatment methods of breast cancer have evolved over the
past 25 years. The relatively recent introduction of contrast enhanced
breast MR imaging has provided radiologists and clinicians with a power¬
ful new diagnostic tool. This article reviews methods of breast MR imaging
and interpretation and identifies the clinical utility of this modality. Cur¬
rent literature suggests that MR imaging is an exquisitely sensitive tool
for detecting breast carcinoma, making it an excellent tool in specific
clinical situations. Rigorous clinical trials are needed to define the role that
MRI CLINICS OF NORTH AMERICA
MR imaging should play in the diagnosis and management of breast
cancer patients.

Metastatic disease is the most common cause of malignant liver disease in
the United States. This article reviews the current MR imaging techniques
used for evaluating hepatic metastases, the MR imaging appearance of
hepatic metastases, and contrast agents that are available or are being
developed to improve detection of hepatic metastases by MR imaging.
MR Imaging of Hepatocellular Carcinoma 757

MR imaging is useful in the diagnosis and early detection of hepatocellular
carcinoma (HCC). Characteristic findings for overt HCC, a pseudocapsule
and an intratumoral mosaic pattern, are demonstrated better with MR
imaging than with other imaging modalities, such as ultrasonography and
CT scanning. For example, signal intensity on T2 weighted images is
useful in grading the malignancy of hepatocystic nodular lesions; gadolin¬
ium enhanced dynamic MR imaging is essential in evaluating tumor vas
cularity; and, although the spectrum of small hepatocellular nodules is
wide in patients with chronic liver disease, MR imaging also can be helpful
in distinguishing small, overt HCC from other precancerous nodules that
develop in the cirrhotic liver.
MR Imaging of the Adrenal Neoplasms 769

This article reviews the differential diagnosis of and MR imaging findings
in a variety of benign and malignant adrenal masses. Imaging techniques
described include chemical shift imaging to detect the lipid within adrenal
adenomas and the fat within adrenal myelolipomas, and heavily T2
weighted images ( MR hydrography ) to characterize adrenal cysts or the
cystic portions of adrenal masses. The most important application of
adrenal MR imaging is in the characterization of adenomas.

imaging features of the most important pancreatic neoplasms are dis¬
cussed, and an approach to classifying pancreatic masses is described.

High field surface coil MR imaging can detect and characterize a broad
range of benign and malignant renal neoplasms. Techniques for optimal
MR imaging of renal neoplasms are discussed. The MR imaging character¬
istics of adult and pediatric renal neoplasms are presented, with an empha¬
sis on the role of MR imaging in detecting, characterizing, and staging of
renal cell carcinoma, the most common adult renal malignancy.

A range of pediatric tumors in children involve the peritoneal cavity and
retroperitoneum and require imaging for evaluation. This article reviews
the clinical aspects and MR imaging appearances of some of the more
important abdominal tumors, and describes how MR imaging can be used
to evaluate them.
MR Imaging of the Urinary Bladder 853

This article describes the staging, treatment, and prognosis of patients
with urinary bladder cancer. Because it is the most accurate imaging
modality, the role of MR imaging is reviewed, and a comparison of the
normal versus cancerous urinary bladder is illustrated. Patient handling,
the choice of pulse sequences, and contrast agents also are discussed. The
roles of MR imaging and CT scanning in staging urinary bladder carci¬
noma are compared and evaluated. Future developments such as new
sequences, contrast agents, and MR image guided biopsy are considered.
The preferred radiologic approach also is discussed.
MR Imaging of Prostate Cancer 869

Prostate cancer is the most common cancer in men. This article reviews
the current tools used for the diagnosis and management of prostatic
cancer. Discussion emphasizes techniques for endorectal MR imaging,
imaging findings, and the value of MR imaging in this area. The new and
promising technique of MR spectroscopy also will be introduced.
MR Imaging of Female Pelvic Malignancies 887

Recent advances, such as multicoil arrays, fast spin echo, and dynamic
gradient echo imaging have increased the role of MR imaging in evaluat¬
ing the female pelvis. MR imaging provides excellent detail of normal
anatomic structures and unique diagnostic information in patients with
pelvic malignancies. Presently, MR imaging is an underused modality for
staging gynecologic malignancies. This article discusses common gyneco¬
logic malignancies, and emphasizes the role of MR imaging, technical
considerations, imaging features, and MR staging criteria.

Soft tissue tumors pose difficult diagnostic problems for clinicians and
radiologists. Soft tissue lesions can be classified as determinate or indeter¬
minate, based on their MR imaging appearance. Common determinate
lesions include lipomas, ganglions, heterotopic ossification, diabetic myo
necrosis, and bursitis. Indeterminate lesions are those that can not be
identified after analysis of the MR imaging appearance. Determinate le¬
sions usually are treated with observation or excisional biopsy, whereas
indeterminate lesions are further evaluated by needle or open biopsy.
MR Imaging of Musculoskeletal Neoplasm 929

This article discusses current MR imaging techniques for evaluating mus¬
culoskeletal neoplasm, and reviews and updates the authors article pub¬
lished in 1998. The role of MR imaging in lesion detection and staging,
lesion characterization, longitudinal assessment of tumors during radiation
or chemotherapy, and detection of local recurrence are explained.
