As advances in information technology continue to evolve, facial plastic
and reconstructive surgeons should seek to understand and implement the
pertinent technology into their clinical practices. This article reports the
findings of a survey of facial plastic surgeons from the United States and
Canada as to their current integration of computer technology.

As managed care continues to put physicians under financial pressure, an
accurate and organized means of tracking costs and operating an office is
essential for the survival and growth of any practice. Important features for
any practice software package include the ability to transfer existing data
into the new package, generate reports on that data, bill electronically, track
finances, coordinate mailings and manage the schedules of patients and staff.
Acquiring an office management software package requires a significant
financial commitment. An equally significant commitment of time should
go into researching the product.

Photography has long been used for medical documentation. The 35 mm
single lens reflex camera has been the traditional device for photodocumenta
tion in facial plastic surgery practices. The widespread availability of com¬
puter hardware and software, digital cameras, and the Internet has enhanced
the advantages and ease of using digital photography. This article reviews
35 mm and digital photography and summarizes the current options that
are available with either 35 mm or digital cameras that enable processing
of images in a digital format.

Personal computers are an integral component in the creation, storage, and
delivery of graphical presentations. There are numerous options available
now to enhance the presentation with graphics, photographs, animation,
video, and audio clips. A good working knowledge of the multimedia
elements is necessary to incorporate their use in the presentation success¬
fully. Multimedia and sophisticated computer technology should be used
to enhance the presentation of information, and, if used improperly, can
actually detract from the presentation. This article serves to familiarize the
reader with the more common multimedia components and general princi¬
ples for preparing a computer slide presentation.

tation graphics systems available for practical application in surgical prac¬

The Internet is changing the nature of the doctor patient relationship and
how practitioners and patients stay abreast of health care information. This
article reviews recent innovations in facial plastic surgery distance learning
including online journals, teleproctored surgery, online discussion and chat
groups, virtual courses and lectures, electronic textbook of facial plastic
surgery, teleconferencing and telemedicine applications, and surgical simu¬
lators.
Searching the Medical Literature 65

The widespread use of computers and the Internet has made searching the
medical literature easier and more accessible to most physicians. The Na¬
tional Library of Medicine (NLM) maintains MEDLINE, a comprehensive,
cross referenced database of citations to the medical literature covering 1966
to the present. The NLM maintains several other literature databases that
are also available online. In 1996, the NLM began providing free, unlimited
access to MEDLINE to all Americans over the World Wide Web. This article
discusses how to perform effective and efficient literature searches using
MEDLINE and other databases.

The computer can be a powerful ally in clinicians efforts to empower their
patients to make the right individual decisions. The Internet, in particular,
is an extremely valuable resource that enables patients to quickly obtain the
latest information on support groups, therapeutic modalities, late breaking
research, and individual coping strategies. The technology is now mature
enough that every provider should consider incorporating some type of
computer based patient education into his or her practice.

A practice web site will, in the near future, be a major method for physician
and patient communication. Each medical practice will need a web site
for providing information to the general public and communicating with
established and potential patients. This article discusses a 4 year experience
with a patient and practice web site and the lessons learned. In addition to
providing a high quality site, there are important legal and ethical issues
to be addressed.

Patients considering facial plastic surgery commonly seek detailed informa¬
tion about the procedures and about their potential surgeon. Satisfied pa¬
tients and physician colleagues remain the most important source of informa¬
tion for these potential patients; over the last few years, however, patients
have identified the Internet as an additional forum for accessing information
about health care, including facial plastic surgical procedures. In this article,
a survey of Internet sites illustrates the widespread efforts in patient educa¬
tion and in practice marketing that exist on the Web. The facial plastic
surgeon must consider how this technology meshes with his or her practice.

Clinical outcomes research has evolved as an attempt to better measure the
patient experience and for this information to be applied to the analysis and
improvement of patient care. For facial plastic surgery to demonstrate its
place in improving the lives of its patients and in improving quality of life,
more studies that document and analyze these changes are needed. This
article introduces the reader to the language and scope of outcomes research
and describes its application to facial plastic surgery.
Computer Aided Craniomaxillofacial Surgery 97

Computer aided surgery (CAS) systems encompass computer enabled diag¬
nostic imaging review and intraoperative surgical navigation. The use of
CAS software tools for the review of preoperative imaging facilitates three
dimensional understanding of complex anatomic relationships. As a result,
CAS can play an important role in surgical planing for craniomaxillofacial
surgery procedures. CAS also permits direct correlation of intraoperative
anatomy with preoperative images; as a result, reconstructive surgeons
may directly relate preoperative image analysis to the actual surgical field.
Despite its impressive power, significant challenges confront current CAS
systems. Fortunately, rapid technological process for CAS is anticipated.
Advanced Surgical Technologies for Plastic and
Reconstructive Surgery 107

Virtual reality creates a simulated world in which the surgeon can practice
and plan complex surgical procedures. This simulated world can be espe¬
cially valuable in head and neck surgery, including congenital craniofacial
surgery, facial trauma, and resection and reconstruction of head and neck
tumors, whereby the complexity of the anatomy and the difficulty of the
problems test the surgeon s abilities. The author presents a brief history of
simulations and surgery, a background to plastic surgery simulators, and
discusses the problem of limited human body models in past systems.
Computer aided surgery and virtual reality technologies are also reviewed.
