This article reviews the techniques of motor and sensory nerve conduction
studies and needle electromyography methods, which arc particularly
useful for localizing nerve injuries in upper extremity and hand trauma.
Included are details of methods for detecting and quantifying the degree
of axon loss and for using this information to make treatment decisions
and predict outcomes. The epidemiology and classification of traumatic
peripheral nerve injuries, the effects of these injuries on nerve and muscle,
and the means by which electrodiagnosis is used to help classify the injury
are described. An overview of recovery mechanisms also is presented.
MR Nerve Imaging of the Wrist and Hand 13

Peripheral nerve imaging is still in a relatively early phase of development.
This article explains a few of the important technical aspects of peripheral
nerve imaging and reviews the results of animal experiments that form
the foundation for interpreting image results. The clinical discussion fo¬
cuses on the use of MR imaging for patients with carpal tunnel syndrome,
keeping in mind that many aspects of the imaging technique may be
applied to other areas. The reliability, diagnostic accuracy, and potential
indications for peripheral nerve imaging are discussed.

physiology from a simple monitor of nerve function to an electrophysio
logic aid to the surgeon. Intentional application of these techniques serves
to achieve the goals of monitoring and guiding the surgical approach,
identifying and localizing nerves, and assessing nerve function. Intraoper
ative neurophysiology provides the surgeon with pertinent information
regarding the functional condition of the nerves under investigation. This
information may be used to clarify and adjust the surgical technique and
optimize postoperative outcome.
Repair of Peripheral Nerve Defects in the Upper Extremity 37

This article presents an algorithm for the repair of peripheral nerve defects,
which ultimately leads to end to end coaptation of the injured nerve. The
end to end coaptation of nerve ends for the repair of peripheral nerve
defects may be accomplished by using one of four techniques. These
techniques (in situ mobilization, nerve rerouting and transposition, joint
positioning, and bone shortening) are discussed in their application to
commonly encountered peripheral nerve injuries. Recommendations are
listed for the repair of segmental peripheral nerve defects in the acute set¬
ting.

This article reviews the anatomy of the peripheral nerve, the pathophysiol
ogy of nerve injury, and Wallerian degeneration. It reviews the factors for
deciding on immediate or delayed primary nerve repair and discusses the
concept of longitudinal excursion of peripheral nerves about joints and
the techniques for achieving an appropriate tension free repair. The tech¬
niques of primary nerve repair, epineurial repair, and group fascicular
repair are reviewed along with techniques for matching fascicles intraoper
atively.

The selection of any surgical technique should be made based on a com¬
parison of the results of the various treatments available. This article
discusses the history of primary nerve repair, the timing of repair, and the
location of the injury. Careful application of the principles of nerve repair
can ensure that the appropriate method of primary, secondary, or nerve
graft is selected. This may be the best way to increase the chances for a
successful result.

viable option in the management of nerve injuries. With modern tech¬
niques, patients now can expect useful recovery of sensory and motor
function following nerve grafting of peripheral nerves. By providing stan¬
dardized and quantifiable data, investigators may combine their clinical
series to identify factors that further enhance nerve repair.

As microsurgical techniques improve, attempts to restore function after
severe nerve injury place greater demands upon the use of nerve grafts.
Viable Schwann cells within these grafts are necessary to maximize nerve
regeneration. The use of transplanted nerve allografts remains experimen¬
tal, yet efforts to understand the host rejection response, advances in
immunotherapy, and the development of neurotrophic factors continue to
reveal significant benefits over standard treatment methods.

Conventional nerve grafting has been a reliable and effective means of
repair to overcome the neural gap, but it is associated with donor site
deficit. The search for the optimal nerve conduit, therefore, has continued
for decades. This article presents a short historical overview of the use of
conduits for peripheral nerve repair including autogenous, biologic, and
synthetic models. The author also speculates about possible future devel¬
opments in peripheral nerve repair and regeneration.

able. Nerve transfers in the upper extremity are important techniques in
the management of many types of peripheral nerve injury. The physiologic
principles of nerve transfer lead to the indications for use. The elements
of planning and execution of common nerve transfers are presented in
this article.

Partial nerve injuries in the upper extremity require accurate diagnosis.
Management of these injuries demands microsurgical skill and precise
instrumentation. Repair is more difficult in chronic injuries, which makes
immediate diagnosis very important. Careful patient selection, knowledge
of topography of the injured nerve, and careful matching of the injured
fascicles helps in obtaining a satisfactory outcome that will allow the
patient to regain a useful function of the injured extremity.

In recent years, there has been a resurgence of end to side peripheral nerve
repair. This technique offers a management of a peripheral nerve defect in
the absence of a suitable proximal stump. Although numerous animal
laboratory investigations demonstrate motor and sensory functional recov¬
ery without deleterious effects to the donor nerve, clinical outcomes are
yet to be determined.
