Pneumonia is the most common cause of death due to infectious disease in the United States and
the most common cause of death in the world. Community acquired pneumonia occurs in the
outpatient setting and usually has a low mortality if hospitalization can be avoided. Hospital acquired
pneumonia and ventilator associated pneumonia have a high incidence as well as high mortality and
morbidity. Nurses are key to identifying patients at risk and introducing interventions that can reduce
or prevent infections.
Noninvasive Respiratory Monitoring 423

Advances in technology now permit a variety of noninvasive respiratory monitoring options for
clinicians. Perhaps a more complex issue is determining how much monitoring is needed as part of

Oxygenation profiles are useful tools available to the bedside critical care nurse. Assessment and
management of critically ill patients is enhanced by the use of oxygenation profiles. This article
reviews the concepts of both tissue and pulmonary oxygenation profiles. Formulas calculated for the
profiles are explained. In addition, the authors provide clinical examples of oxygenation profile
application throughout the article.

Mechanical ventilation can be life sustaining in the critically ill patient; however, there is much data
to confirm that mechanical ventilation can also damage the lungs. Although there is no concrete
evidence that will allow us to prescribe the ultimate ventilation strategy, research in this area is
ongoing. This article reviews current lung protective strategies and their implications for the critical
care nurse.

Human Immunodeficiency Virus Disease: Managing Respiratory

Increasing numbers of individuals are being admitted to the critical care unit with respiratory complica¬
tions related to human immunodeficiency virus (HIV) disease. Respiratory complications affect both
pediatric and adult patients. The critical care practitioner must have knowledge of these pathogens
to provide the most comprehensive care possible and improve patient outcomes. This article reviews
the primary pulmonary complications observed in children and adults who are HIV positive: pneumo
cystis carinii pneumonia, mycobacterium tuberculosis, bacterial pneumonia, and lymphoid interstitial
pneumonia/pulmonary lymphoid hyperplasia. The clinical presentation of these complications is
discussed along with the current collaborative management.
Making Weaning Easier: Pathways and Protocols That Work 465

Weaning patients from prolonged ventilation can be difficult, time consuming, and costly. As a result,
institutions, as well as the clinicians who are charged with the care of patients, struggle to determine
quality solutions. Increasingly, critical pathways and weaning protocols are being used to accomplish
this goal. This article describes the science related to the use of pathways and protocols for long
term mechanically ventilated patients and how they may be applied clinically to attain quality outcomes.
Respiratory Failure: After the Intensive Care Unit 481

Acute or chronic respiratory failure can result in a complex series of events. A patient may experience
multiple sites of care as he or she progresses from critical illness to recovery. Complete recovery
with a return to pre hospital function status and discharge home may not always be an achievable
outcome. Decisions regarding where care can be delivered are influenced by organizational policy,
availability of resources, and insurer requirements and contracts. Many patients and families are not
prepared for the transition of care from site to site, especially away from the acute hospital. This
article examines alternative sites of care after the intensive care unit and includes strategies for patient
and family education and counseling in preparation for transition of care.
State of the Art: Neonatal Respiratory Failure 493

Tremendous advances in the management of neonatal respiratory distress, which have dramatically
decreased the morbidity and mortality of the neonatal patient, have been made during the last three
decades. The technological explosion of the 1990s resulted in the adoption of several innovative
techniques for neonatal ventilation. This article discusses the clinical application, adverse effects, and
nursing implications of four of these treatment options: patient triggered ventilation (PTV), high
frequency ventilation (HFV), liquid ventilation, and nitric oxide (NO) therapy.
Ventilatory Strategies for the Critically III Infant and Child 501

Numerous advances have been made in pediatric ventilatory strategies in recent years. There are
now many different therapeutic options available to the pediatric critical care practitioner. In order
to choose the most appropriate ventilatory strategy, practitioners must be familiar with the risks and
benefits of each of their options. This article reviews various modes of mechanical ventilation and
adjuncts to ventilation.

Asthma is the most common chronic illness in children and is a leading cause of illness associated
morbidity. Hospitalization rates and deaths due to asthma in children have been steadily rising despite
a greater understanding of and better treatment for this disease. Status asthmaticus can progress to
respiratory failure and death if not treated promptly and appropriately. Nurses working with children
in the critical care stetting need an understanding of the pathophysiology of status asthmaticus, risk
factors associated with its development, and common treatment modalities used.

When the Lung Fails: Acute Respiratory Distress Syndrome in

Acute respiratory distress syndrome (ARDS) is the most severe end of the clinical spectrum of acute
lung injury. This syndrome affects many children admitted to the pediatric intensive care unit and
the mortality rate is still high. Caring for the child with ARDS provides unique challenges for the
critical care nurse. A variety of ventilatory strategies have been introduced to improve survival rates
and decrease morbidity; however, regardless of the therapy, expert nursing care is critical to maximize
a good outcome. This article reviews the pathophysiology of ARDS, examines assessment parameters,
and discusses current trends in management with nursing implications. Supportive interventions to
optimize outcomes are also addressed.
