Glucocorticoid use may unmask hyperglycemia in patients with¬
out known diabetes mellitus and increase hyperglycemia in pa¬
tients with known diabetes mellitus. This article summarizes pos¬
sible mechanisms of glucocorticoid induced hyperglycemia, the
prevalence and magnitude of the hyperglycemic effect of gluco
corticoids and the rationale for acute and chronic glycemic con¬
trol. Strategies for the management of hyperglycemia are based
on the glucose lowering management in place before the use of
glucocorticoids (diet, oral agents, or insulin), the glucocorticoid
doses being given, and the degree of hyperglycemia are dis¬
cussed.
Management of Lipid Disorders 507

Atherosclerotic cardiovascular disease and lipid disorders are
common in patients with rheumatologic disorders. Plasma lipids
should be screened and lipid disorders actively treated in an
attempt to reduce cardiovascular risk in this population. Care
should be taken to avoid side effects and drug interactions, which
may be somewhat more likely to occur in rheumatolotic patients.

Hypertension is prevalent and strongly associated with the devel¬
opment of cardiovascular disease. The current recommendations

for treatment of hypertension according to level of risk are exam¬
ined. Nonpharmacologic and pharmacologic therapy are outlined.
In particular, treatment recommendations and drug choices in the
setting of diabetes mellitus, peripheral vascular disease, asthma,
Raynaud s, gastroesphageal reflux disease, and renal insufficiency
are reviewed. Effects on blood pressure of drugs used in rheuma
tologic practice are discussed.
Women s Health 539

There is growing interest in women s health as a discrete new
field in medicine. This interest is fueled by the recognition that
women have unique health issues and that some diseases affect
women in specific and unique ways. Women s health is multidis
ciplinary, incorporating topics traditionally included in internal
medicine, obstetrics and gynecology, and even geriatrics. Wom¬
en s health incorporates many topics, only a few of which are
addressed here. In this article, consideration is given to contracep¬
tion, pregnancy, and menopause, both in general and in women
with autoimmune disease. Osteoporosis is not discussed in detail
because it is covered extensively in the rheumatology literature.
It is essential that rheumatologists have expertise in these areas
because they often provide primary medical care for women with
active rheumatologic disorders.

immunocompromised patients. This article reviews the data on
efficacy and safety of currently licensed vaccines in patients with
rheumatologic diseases.
Recognition and Management of Preoperative Risk 585

The major role of the internist in the perioperative period is to
identify and quantify a patient s risk for surgery, to minimize this
risk, to monitor for problems, and to treat problems as they
occur. A careful history and physical examination with only very
selective laboratory testing and imaging is indicated. Cardiac
issues predominate. An algorithmic approach that uses risk indi¬
ces and selective interventions is presented. Other common prob¬
lems include pulmonary diseases, diabetes, anemia, and specific
concerns in the elderly and those with rheumatologic diseases.
This article reviews a practical approach to the evaluation and
treatment of these issues as well.

Patients undergoing surgery are subject to multiple perioperative
problems. This article reviews several issues that occur in surgical
patients with rheumatic diseases, including management of medi¬
cations, diagnosis of fat embolism syndrome, prophylaxis against
endocarditis, postoperative fever, and perioperative myocardial
infarction.

Joint replacement surgery is one of the most frequently performed
procedures in the United States. The incidence of deep vein
thrombosis and pulmonary embolism is very high in patients not
receiving prophylaxis for the prevention of this postoperative
complication. In this article, the current modalities for prophylaxis
are reviewed with respect to their safety and efficacy. Recommen¬
dations that have been substantiated by evidence based informa¬
tion are provided.
Detection and Management of Coronary Artery Disease

Patients with rheumatologic disorders can present with a variety
of nonspecific symptoms, such as chest pain, fatigue, or dyspnea,
that could represent significant coronary artery disease. The indi¬
cations for noninvasive stress testing that can help differentiate
deconditioning from this serious common disorder are reviewed.
Dobutamine stress echocardiography and dipyridamole thallium
imaging studies, the two most commonly used noninvasive tech¬
niques in identifying and localizing coronary artery disease, are
compared and contrasted, and an approach to coronary artery
disease management is presented.
Large Artery Occlusive Disease 669

Large artery occlusive disease is a common problem in the United
States. It affects both the upper and lower extremities and is
associated with significant morbidity and mortality. This article
deals with the clinical recognition of this entity in hopes that the
general internist and rheumatologist will more easily recognize
it. In addition, the latest technology available to diagnose and
treat large artery occlusive disease is discussed.

Depression is commonly seen in patients with chronic disease,
including rheumatologic disease. Because of ongoing contact with
their patients, rheumatologists are uniquely poised to recognize
depressive disorders and formulate a treatment plan. Recent ad¬
vances in pharmacotherapy have simplified management and
increased the likelihood of successful treatment. Through height¬
ened awareness, rheumatologists can feel more comfortable in
diagnosing and initiating therapy for depression.
Dyspepsia and Heartburn 703

Dyspepsia and heartburn are the two cardinal symptoms of fore
gut dysfunction. When confronting such a problem, that physi¬
cian must first learn to discern between the two, because treat¬
ment can be quite different for the conditions presenting with
these symptoms. This article details the approach to work up and
treatment of patients presenting with dyspepsia or heartburn.
Screening for Cancer in the Patient with Rheumatic Disease 719

This article begins with a discussion of the types of cancer that
are most often the target of cancer screening in general medical
practice. Colorectal, prostate, cervical, and breast cancer screening
guidelines are reviewed and some of the evidence (and contro¬
versy) behind these recommendations is discussed. A discussion
of some of the special risks for cancer in patients with rheumato¬
logic disease follows, and some modifications of standard screen¬
ing guidelines are suggested for these high risk patients.
Selected Drug Complications and Treatment Conflicts in
the Presence of Coexistent Diseases 745

The presence of different coexistent systemic diseases often times
complicates the selection of the appropriate treatment of an un¬
derlying rheumatologic condition. In this article, some controver¬
sial treatment conflicts that are frequently encountered in the
daily practice of rheumatology are clarified and guidelines for
the best available therapeutic options are provided.
