Contrasts between primary care and psychiatry are examined from the pri¬
mary care provider s view. Tension exists in the relationship where collab¬
oration (especially well demonstrated by somatization) would be sorely
needed. A reconciliation is possible and recommendations are made.

The differing practice patterns of a family physician and a psychiatrist illus¬
trate the complexity of the primary care mental health interface. Though
we have no unitary model for understanding the interface, a scheme is sug¬
gested that can decrease confusion when trying to solve problems between
the specialties.

The mental health provider brings expectations to the collaboration with
primary care. These include equitable coverage for mental disorders and
access to primary care. Beyond this, a reduction in interference with each
other s practice and a willingness to collaborate for the good of the patient
are required.

An understanding of the characteristics of a primary care practice sets the
stage for smooth collaboration. Also required is easy access to mental health,
information sharing, and an interest to help even beyond traditional prior¬
ities.

Integrated systems in the public sector require an understanding of various
consultative models. Increased access and communication can help develop
structures that support the funding of these critical services.

In carved out systems, the coordination of care has top priority. Barriers
must be systematically sought and eliminated. Who provides services, how
the providers communicate, and who pays for the services are critical issues.

Gatekeeping and authorization are managed care techniques increasingly
seen in the public sector, particularly where the primary care physician is
responsible for specialty costs. Difficulties with this process can affect diag¬
nosis, patient acceptance of referrals, effective treatment, and payment.

The key to successful collaboration is effective communication between
providers for the good of the patient. An understanding of the differing lev¬
els of confidentiality is crucial to managing requests for information, access
to records, and transmission of information.

Both the primary care and the mental health view add value to emerging
public systems of care. Financial, administrative, and clinical considerations
must be addressed. Opportunities for collaboration result, including coor¬
dinated quality improvement.
