This article focuses on both static field effects and oscillating magnetic fields
associated with MR imaging and spectroscopy. Contemporary experiments
and theories on health effects demonstrate that currently MR imaging is
practiced in a safe manner. Technological capabilities and medical science
objectives, however, will lead to procedures that will challenge the thresh¬
olds of physiological effects. Thus progress in this field will require continual
surveillance and better definitions of guidelines which at present are consid¬
ered prudent but too restrictive.
MR Safety at High Magnetic Fields 715

MR imaging requires the exposure of human patients to magnetic fields that
are much more intense than ever occur naturally. Under most circumstances,
however, these fields do not appear to pose a health or safety risk even at
levels well above those currently in clinical use. This margin of safety results
from the extreme weakness of the diamagnetic interaction between human
tissues and magnetic fields. Ferromagnetic materials, however, present ei¬
ther as implants within the patient or inadvertently introduced into the
scanner environment, pose significant risks and must be scrupulously
avoided. At very high field strengths, there is evidence for mild sensory
effects, such as vertigo and metallic tastes, which do not appear to be
harmful.

At sufficient amplitude, gradient induced electric fields may stimulate
nerves and muscles in patients during MR examinations. This article reviews
the physics, physiology, and safety aspects of nerve and muscle (including
respiratory and cardiac) stimulation. This article also reviews experimental
data on gradient induced stimulation. While threshold stimulations are just
perceptible, patients may experience pain at high electric field amplitudes.
Safety margins required to prevent cardiac stimulation and to limit painful
stimulations are investigated.

There is uncertainty and controversy about the extent to which low level elec¬
tromagnetic fields may cause deleterious effects, but even experts who are
skeptical about many supposed hazards are willing to agree that electromag¬
netic fields even weaker than those in the MR environment can have effects
under certain conditions. In order that readers can familiarize themselves
enough with the subject to make an informed independent assessment, dis¬
cuss it knowledgeably in public, and have the means with which to evaluate
new developments and avoid experimental pitfalls if planning their own re¬
search in the area, they are provided with some of the most recent finding
of in vitro and in vivo research from outside the MR literature as well as
some of the results and controversies coming from recent epidemiological
studies.

Radio frequency fields used in MR imaging may result in patient heating.
This article reviews the physics, physiology, and safety considerations of
radio frequency heating of patients during MR examinations. Frequently,
monitoring equipment, implants, braces, surface coils, or other devices are
introduced into the patient vicinity. A section investigates safety aspects of
such devices. Appropriate limits for radio frequency power deposition dur¬
ing MR examinations are explored.
FDA Regulation of the Safety of MR Devices: Past, Present,
and Future 791

The history of the Food and Drug Administration s (FDA) regulation of
safety issues for MR devices is reviewed in this article. The guidance docu¬
ments and policies that FDA has used from 1982 to the present to ensure
safety are summarized. The present status of FDA regulatory policies and
apparent plans for the future are included.

Cortical mapping is an important adjunct to the workup of patients with
arteriovenous malformation (AVMs) near eloquent cortical regions. Task
activation imaging can be performed that clearly identifies primary cortical
regions. Although current methods require considerable postprocessing,
advances in hardware and software will likely make functional MR imaging
a routing examination before surgical resection of AVMs in the near future.
Magnetic Resonance Angiography of Vascular Malformations 811

The rapid advancement of MR angiography techniques over the past few
years has brought this imaging modality to the forefront in the diagnosis
and management of vascular diseases of the central nervous system. The
ability to acquire a three dimensional dataset in a single acquisition and
display these data using a number of different postprocessing techniques has
made MR angiography especially useful in the evaluation of arteriovenous
malformations (AVMs) before radiosurgery. In many instances, MR angiog¬
raphy has replaced conventional angiography for the purpose of radiosur¬
gery treatment planning and follow up. As this technology continues to
advance, it will undoubtedly play an even more important role in the
management of vascular malformations in the future.
Magnetic Resonance Imaging of Intracranial Aneurysms 835

Intracranial aneurysms are lesions with high morbidity and mortality, but
in most cases represent treatable disease. Magnetic Resonance (MR) imaging
and in some cases MR angiography can make valuable contributions to
their diagnosis and characterization. The ultimate tool for imaging these
lesions, however, remains catheter angiography. This article focuses on
saccular aneurysms, with a brief discussion on atherosclerotic fusiform aneu¬
rysms and mycotic aneurysms.
Dedicated Extremity MR Imaging: An Emerging Technology 849

Dedicated extremity MR imaging represents a radical departure from con¬
ventional whole body scanning. Extremity MR imaging offers such advan¬
tages as reduced cost, more convenient and inexpensive setting, greater
patient comfort and safety, and high diagnostic power. This article examines
some of the features of extremity MR imaging and how this technology is
affecting musculoskeletal imaging in today s environment of cost contain¬
ment and health care reform.
MR Arthrography of the Hip 871

This article discusses the use of MR arthrography for the evaluation of
mechanical derangement of the hip. This technique is used primarily to
evaluate the acetabular labral tears; however, the articular cartilage and
joint capsule also are visualized. MR arthrography and hip arthrography
have had a significant impact on the diagnosis and treatment of patients
with mechanical hip pain.
MR Arthrography of the Ankle 885

Injuries to the lateral ankle ligaments are extremely common. Many patients
with acute injuries to the lateral ligament complex develop chronic ankle
instability that requires surgical intervention. MR arthrography of the ankle
plays a valuable role in identifying those patients who are most at risk for
the development of chronic instability. MR arthrography of the ankle also
aids in preoperative planning and in differentiating among other entities
that can present as ankle instability.
