Patients who suffer from chronic rheumatologic diseases, such
as rheumatoid arthritis, frequently require orthopedic surgical
intervention during the course of their illness. This article pro¬
vides the reader with an overview of approaches to postoperative
risk stratification. Reviewed are the basic concepts that underlie
perioperative medical management, including such issues as the
preoperative medical assessment, the currently employed anes¬
thetic techniques, and approaches to postoperative analgesia. The
impact of comorbid conditions on surgical outcome is discussed
as are specific clinical problems that have particular relevance to
the patient with rheumatoid arthritis.
Anesthesia on the Rheumatoid Patient 19

In the orthopedic setting, the anesthesiologist often encounters
the rheumatoid patient after the disease has run a resistant
course. Despite optimal medical therapy, the patient at this point
has intolerable levels of pain or limitation of function because of
structural joint damage. Given the duration of the disease and
severity of symptoms, a number of pharmacologic modalities
already have been tried for which the patient has suffered some
side effects. Now that the disease has spanned a significant pe¬
riod, the patient has become older with one or more coexisting

illnesses. These comorbidities may occur independently or as part
of the extra articular manifestations of the underlying rheumatic
disorder. Because of these reasons, rheumatoid patients today
present major anesthetic challenges.

Many rheumatoid patients continue to have pain and progressive
deformity despite a thorough attempt at medical management
and require orthopedic intervention. The rheumatoid patient
commonly presents to the orthopedist with multiple orthopedic
problems. It is the responsibility of the orthopedist, working in
conjunction with the rest of the patient s care givers to formulate
a plan for sequentially addressing the surgical needs of the rheu¬
matoid patient. The considerations given to the formulation of
this plan are discussed within this article.

Rheumatoid arthritis most often affects the cervical spine, re¬
sulting in clinical and radiographic findings. For most patients
with rheumatoid arthritis the cervical involvement represents a
relatively benign process, but, in a small percentage of these
patients, a progressive instability pattern develops that may com¬
promise neural or vascular structures. The rate of neural compro¬
mise because of cervical instability ranges from 11% to 58%.
The clinical manifestation may be radiculopathy, myelopathy,
quadriplegia, and, in extreme instances, sudden death.

Rheumatoid arthritis of the glenohumeral joint can produce sig¬
nificant pain and disability that interferes with the ability to
perform even the basic activities of daily living. In this article the
authors discuss the epidemiologic aspects of rheumatoid arthritis,
the pathologic condition as it affects the shoulder complex (con¬
sisting of the glenohumeral, acromioclavicular, and sternoclavicu
lar joints), the differential diagnosis, clinical and radiographic
manifestations, and treatment approaches designed to maintain
or regain function.

evolve. This article discusses current surgical treatment options,

This article discusses the wrist and hand deformities associated
with rheumatoid arthritis and the treatment options available to
the patient. The goals of surgical treatment of this disease are to
reduce pain, retard progression of the disease, and preserve or
restore form and function. Considerations include conservative
therapy, indications for surgery, and expected outcomes.

Total hip arthroplasty (THA) has improved significantly in the
last 30 years since the popularization of the procedure by John
Charnley in the late 1960s. The significant advances in THA in
the last 20 years have been that it is highly reproducible and
durable, has high quality of function and low morbidity, patient
satisfaction is high, and salvage is possible when necessary. The
main object of this article is to discuss the surgical management
of patients who have RA of the hip joint, dealing with indications,
surgical techniques, results, and complications.
The Knee Joint in Rheumatoid Arthritis 143

The knee joint is commonly damaged in polyarticular rheumatoid
arthritis. Surgical management depends on the preservation of
the articular cartilage and degree of synovitis. Arthroscopic syno
vectomy is performed in intractable synovitis with cartilage spar¬
ing. Technique of total knee replacement is more complex because
of bone and soft tissue damage in rheumatoid arthritis, and com¬
plications are more common, particularly infection. Overall, long
term results with knee arthroplasty have been excellent in rheu¬
matoid arthritis, with improved function and reduction in pain
in the vast majority of patients.
Rheumatoid Arthritis of the Ankle and Hindfoot 157

Rheumatoid arthritis frequently affects the hindfoot and ankle
and may present a considerable source of dysfunction. Awareness
of characteristic clinical and radiographic findings and other diag¬
nostic modalities help the clinician to evaluate the progression of
the disease and determine the best methods of treatment. A
variety of nonoperative treatments may slow progression of de¬
formities, improve function, and provide symptomatic relief. If

these measures fail, surgical intervention, including soft tissue
procedures and a variety of arthrodesis techniques, can return
patients to a more active lifestyle.
Rheumatoid Arthritis of the Forefoot 173

Severe and often debilitating involvement of the forefoot is seen
frequently in patients with rheumatoid arthritis. The combination
of destructive synovitis and weight bearing leads to a spectrum
of disorders causing pain, deformity, and eventual loss of func¬
tion. In addition, advances in total knee and total hip arthroplasty
have placed added importance on preserving the ability of the
foot and ankle to bear weight and allow functional ambulation.
This article reviews the epidemiology, pathophysiology, clinical
presentation, physical examination, and treatment of rheumatoid
arthritis of the forefoot.
General Overview of Rehabilitation in the Rheumatoid

This article provides a general overview of rehabilitation in the
rheumatoid patient. It discusses the physical therapy manage¬
ment of both the systemic musculoskeletal involvement of RA
and of the specific postoperative rehabilitation associated with
total joint arthroplasty. It is intended to highlight the major com¬
ponents of treatment and to guide the clinician in assessing these
challenging, yet rewarding cases. Its emphasis is to support a
multidisciplinary approach to patient care.
