Rejuvenation of the Upper Face: A Logical Gamut of
Surgical Options 199

Despite advances in techniques, aesthetic surgeons must realize that not
every patient requires a complete forehead rejuvenation. This article dis¬
cusses the contemporary armamentarium for forehead rejuvenation.
Injection of botulinum toxin for glabella furrows is minimally invasive but
only temporarily effective. Fat injection and fat grafts are useful for pa¬
tients with a limited number of wrinkles and frown lines. Subcutaneous
brow elevation provides direct correction of the forehead wrinkling by
dividing the subcutaneous septae responsible for the furrows. Endoscopic
techniques facilitate forehead rejuvenation with less morbidity and greater
patient acceptance. Detailed descriptions of the spectrum of surgical op¬
tions for forehead rejuvenation are included.

The endoscopic facial rejuvenation approach allows for repositioning of
the forehead, brows, and periorbital and malar tissues. Through minimal
incisions and using magnification for soft tissue dissections, the endo¬
scopic approach allows surgeons to reach areas that are otherwise difficult
to approach with the open face technique, maintaining a normal plane of
cleavage and protecting structures (motor branches of facial nerves) while
allowing wide soft tissue mobilization. These procedures are indicated for
forehead periorbital and upper face rejuvenation with minimal to moder¬
ate improvement of the jaw area. It can be performed alone or in
conjunction with the open approach for correcting lower face and neck
aging. Indications, advantages, and complications are discussed in this ar¬
ticle.

The evolution of the temporal mid facelift (extended browlift) from an
open hemicoronal approach to a limited temporal endoscopic procedure
is reviewed. The procedure emphasizes a lateral brow elevation, a vertical
suspension of the malar retaining ligament, and a temporal reshaping of
redundant skin.

This article outlines the author s philosophy on facial aging that he uses in
a surgical technique that is unique because it requires no SMAS tightening,
incision above the auricle, deep plane dissection, subperiosteal dissection,
or stitches in the face above the mandibular border. The fatty nasolabial
and labiomandibular prominences are excised using sharp dissection or
curette to flatten these areas. Various case studies with extensive follow
up are provided. In the author s experience, this surgical technique has
shorter recovery time, higher patient satisfaction, fewer complications,
excellent longevity, and a more natural aesthetic result than deeper and
more extensive techniques.

Experience has shown that a superior facelift result is not possible without
modification of the SMAS, platysma, and other deep facial structures.
Although techniques vary and a consensus of opinion is unlikely to
emerge, any surgeon able to identify the anatomic basis of his or her
patients problems can, through the application of logic and careful plan¬
ning, select appropriate methods that are safe, effective, and rational.
Presented here is the author s technique, which has delivered satisfaction
to many patients.

It is well known that the aging process affects all tissues and does not
occur exclusively in some of them. Thus, the surgical procedure of choice
to restore the face is the one that corrects the alterations at the different
anatomic planes and their structures or traits. The purpose of the subperi¬
osteal facelift is to relocate the deep soft tissues of the face while allowing
treatment specifically and separately of the flabbiness of the deep soft
tissues and the skin.

In most patients, the upper portion of the subcutaneous fatty mass—the
malar pad—is modest in size and only moderately displaced inferiorly.
Mobilization of the malar pad together with the ovelying skin flap achieves
a pleasing result in these cases. Occasionally a patient will present with
very prominent and ptotic malar fat pads and deep nasolabial folds that
cannot be adequately addressed by existing methods. This article describes
a technique of malar pad suspension by direct suturing that preserves the
blood supply to the pad while placing it in a higher, more aesthetic posi¬
tion.

Composite rhytidectomy is an integrated facelift technique that has been
developed with the philosophy that every area of the aging face undergoes
topographic changes that are created by changes in deep anatomic struc¬
tures. The goal of harmonious facial rejuvenation is youthful contours of
every area of the face, including an elevated forehead and a youthful neck
contour. Most importantly, the midface must have a narrow, more shallow
orbital appearance to be compatible with an improved nasolabial fold and
jawline. There have been several refinements in technique, outlined in this
article, that have contributed to consistent and dependable results.

This article presents a specific method of autologous fatty tissue trans¬
plantation, Lipostructure, which incorporates the technique of syringe
liposuction with an intricate layering of autologous fatty tissue. The pre¬
sented methods of infiltration of fatty tissue allow precise control over the
contours of the face to replace atrophied or missing structures and enhance
facial contours. Autologous fatty tissue harvested, refined, and placed in
the specific fashion described is presented as an exemplary agent for
augmentation in soft tissue facial recontouring.

Aesthetic Skin Resurfacing with the High Energy

The introduction of the high energy, short duration pulsed CO2 laser has
made possible the resurfacing of skin in patients with actinic damage,
rhytids, acne scarring, and exophytic skin lesions. CO2 lasers have enjoyed
extensive use in dermatology, although their applications were limited
by nonspecific thermal effects. These problems have now been largely
eliminated with the advent of the high energy ultrapulsed CO? laser sys¬
tems. High energy ultrapulsed CO2 lasers may offer significant advantages
over other resurfacing techniques.

Laser resurfacing with the new ultra fast pulsed CO2 laser has advanced
the ability to rejuvenate skin. Resurfacing with the laser has advantages
and disadvantages over chemical peeling. Physicians need to weigh the
benefits and risks of each resurfacing modality before deciding what is
best for a particular patient.
