The many testing methods available and the increasing sophisti¬
cation of diagnostic equipment have enhanced greatly our capa¬
bilities to diagnose causes of exercise intolerance in the equine
athlete during the last several years. High speed treadmill exami¬
nation has become the focus of this form of evaluation. Not all
clinicians perform or have access to high speed treadmill exami¬
nations. Testing methods that require the use of the high speed
treadmill, as well as methods that do not, are discussed.
Upper Respiratory Causes of Exercise Intolerance 435

The upper respiratory system is a frequent cause of exercise
intolerance in performance horses. Labored breathing, fatigue
during performance, and prolonged recovery following exercise
are common presenting complaints. Inadequate fitness level and
diseases of the cardiovascular system are differential diagnoses
that share these complaints and should be ruled out. Generation
of increased airway noise is a clinical sign that implicates the
upper respiratory system. A careful history from the owner and
endoscopy of the upper airway are the most useful diagnostic
tools. Endoscopy during exercise on a treadmill is indicated when
an upper airway lesion is suspected but not apparent with endos¬
copy at rest. Treatment of respiratory obstruction may vary with
the level of performance required of the horse.

Lower respiratory tract disease is an important source of poor
performance and exercise intolerance in racehorses and middle
aged sport horses. Horses that perform high intensity exercise
are predisposed to development of infectious and noninfectious
respiratory disease. Diagnostic aids for investigation of lower
respiratory tract disease include thorough thoracic auscultation
with rebreathing, endoscopic examination, bronchoalveolar la
vage (BAL), and thoracic radiographic examination. The thera¬
peutic approach for horses with lower respiratory tract diseases
often can be directed by cytologic evaluation of BAL fluid. Con¬
servative management techniques may reduce the risk or severity
of respiratory disease in horses performing high intensity exer¬
cise.
Cardiovascular Causes of Exercise Intolerance 473

Exercise intolerance, due to cardiovascular disease in horses, may
be caused by cardiac arrhythmias, valvular regurgitation, congen¬
ital abnormalities, myocardial dysfunction, pericardial disease,
and vascular thrombosis. The most common cardiovascular cause
of exercise intolerance in horses is atrial fibrillation. Cardiovascu¬
lar abnormalities such as cardiac arrhythmias or murmurs, how¬
ever, are common in athletic horses and are not always associated
with exercise intolerance. Use of an electrocardiography (during
rest and exercise) and echocardiography may be necessary to
better determine the significance of the cardiovascular abnormal
ity.
Muscular Causes of Exercise Intolerance in Horses 495

The muscular system of the horse is remarkable in its athletic
scope and capacity to adapt to the demands placed on it. Muscu¬
lar fatigue often causes exercise intolerance in horses as a primary
muscular dysfunction or secondary to abnormalities in other inte¬
grated systems such as the cardiovascular, respiratory, or skeletal
system. This article reviews basic muscular physiology leading to
a discussion of the physiologic causes of muscular fatigue. In
addition, a review of pathologic muscle disorders such as muscle
strains and exertional myopathies that lead to poor performance
is provided.

Skeletal origins of exercise intolerance or performance failure
most commonly are shown as lameness. Subtle gait abnormalities

are occasionally consistent with continued training, but the qual¬
ity of expected competition can be decreased by skeletal tissue
injury. Familiarity with the anatomy, physiology, and biomechan
ical function of equine skeletal tissues is necessary to interpret
the influence of injury on performance. Physical examination,
application of new technologies, and imaging studies often are
needed to understand fully skeletal tissue origins of exercise
intolerance.
Metabolic Causes of Equine Exercise Intolerance 537

Metabolic causes of exercise intolerance often are manifested as
exertional rhabdomyolysis caused by the high rate of energy use
in exercising muscle. Other metabolic causes can be separated
into acid base, electrolyte, endocrine, and heat related effects.
Many metabolic problems are cumulative in heat exhaustion,
particularly in endurance and 3 day event horses. Dietary manip¬
ulations and other ergogenic aids have been suggested in recent
years as a method to provide more available or more easily used
energy substrates for exercising muscle. These ergogenics have
been proposed to enhance the horse s ability to exercise faster
or longer before substrate related fatigue becomes performance
limiting. The efficacy of many of these ergogenics remains un
proven, and some have serious detrimental side effects.
Diagnostic Approach to Exercise Intolerance in Racehorses 555

Exercise intolerance is a common clinical problem in race track
practice. Retrospective data that have been generated from multi
modality treadmill examinations can provide the general prac¬
titioner with a better directed diagnostic approach to this prob¬
lem. The major body systems that are usually responsible for
the exercise intolerance include the musculoskeletal, respiratory,
cardiovascular, and neurologic systems. This article places em¬
phasis on the collection of historical clinical data as well as the
performance of a sound physical examination.

Endurance competition requires synchronism and development
of metabolic and musculoskeletal systems. An understanding of
the existence of performance limiting factors may permit the de¬
tection of exercise intolerance that could lead to performance
failure, fatigue, and exhaustion. New concepts for assessment of
fitness have increased the understanding of individual capacities
and deficiencies and the interaction of the different systems in¬
volved in exercise.

This article deals with the presenting histories, clinical examina¬
tions, and therapies of the causes of poor performance and exer¬
cise intolerance in the western performance horse and the sprint
racehorse. The veterinarian s ability to diagnose and treat various
pathophysiologic conditions that affect these horses is crucial to
the major goals of a return to optimal performance and a long
athletic career. Although these horses are a significant portion
of most veterinary practices, there exists a minimal amount of
information on their clinical evaluation and treatment. This article
intends to fill this void.
Evaluation of Young or Unproven Horses
for Potential Exercise Intolerance 607

The determination of poor performance in the young or unproven
horse can be a difficult experience for the practitioner. This article
describes an incremental exercise test that may be helpful in
determining the cause of poor performance in horses when a
complete physical examination and a lameness examination fail
to elucidate the causes of poor performance. Included are values
from normal yearling and 2 year old horses that have undergone
the incremental exercise test.
