The anatomy and physiology of the normal carotid artery serves as a starting
point for understanding and evaluating existing and new methods for diag¬
nosing carotid artery disease. This article summarizes the history and state
of the art of carotid artery structure and function.
Carotid Atherosclerosis: Definition, Pathogenesis,
and Clinical Significance 801

Atherosclerotic plaques are aggregates of plasma lipids (especially choles¬
terol), cells (smooth muscle cells and monocytes/macrophages), and connec¬
tive tissue matrix (collagen fibers and proteoglycans). Symptomatic plaques
in the carotid artery involve primarily the carotid bulb and are characterized
by increased cellular proliferation, lipids accumulation, calcification, ulcer
ation, hemorrhage, and thrombosis. Risk factors (arterial hypertension, ciga¬
rette smoking, high serum levels of cholesterol, and fibrinogen) promote
thrombus formation and continuous transendothelial seepage of plasma
lipids.
Nonatheromatous Carotid Artery Disease 811

Common and uncommon nonatheromatous diseases affecting the cervical
carotid artery are discussed. Specifically, the radiographic and pertinent
clinical features of cervical carotid artery congenital variants, dissection,
fibromuscular dysplasia, Takayasu s arteritis, and infection are described.

Computed Tomography Angiography 899

Spiral computed tomography angiography is a relatively new noninvasive
technique capable of rapid imaging of the carotid artery. Computed
tomography angiography usually is performed in a 30 second scan time
and images iodinated contrast as it fills the vascular lumen. Studies
comparing computed tomography angiography with conventional angiog¬
raphy show sensitivities and specificities that rival any of the other
noninvasive imaging techniques for the detection of carotid artery stenosis
and occlusion.
Can Noninvasive Studies Replace Conventional Angiography in the
Preoperative Evaluation of Carotid Stenosis? 911

Most articles in 1995 demonstrated a sensitivity of 100% and specificities
of 90% to 98% in detecting carotid bifurcation stenosis using a combination of
MR angiography and ultrasonography, reserving conventional angiography
for cases of disparate stenosis with the noninvasive studies. Multiple reviews
concluded that noninvasive studies were less morbid than conventional
angiography when taking into account the risks of conventional angiogra¬
phy and surgery as well as the morbidity associated with unnecessary
surgery (false positive noninvasive study) or withholding indicated surgery
(false negative noninvasive study). Thus, the current literature suggests that
non invasive studies have indeed reached a point where they can replace
conventional angiography in the preoperative evaluation of carotid bifurca¬
tion stenosis. It is paramount that each imaging center review its own
sensitivity and specificity in detecting surgically significant carotid stenosis
with noninvasive studies. High sensitivity and specificity in diagnosing
percent carotid stenosis with noninvasive studies can occur today with
routine imaging equipment, but it takes a great deal of effort and dedication
on the part of the imaging center.
Carotid Endartectomy Trials 931

For endarterectomy for symptomatic carotid artery stenosis, there is strong
evidence that has been universally accepted and put forward by the North
American Symptomatic Carotid Endarterectomy Trial (NASCET), with cor
roboration from the European Carotid Surgery Trials Collaborative Group,
that patients with severe stenosis have an extremely high risk for stroke
without carotid endarterectomy and a low risk with it. We are awaiting
for NASCET information regarding the lower limit of percent stenosis for
which patients with symptoms should be considered for surgery.
Carotid Endartectomy 939

This article provides an in depth review of carotid endarterectomy for
the treatment of occlusive carotid disease. Topics range from preoperative
diagnostic assessment to postoperative complications. Key issues related
to operative management, including anesthetic technique, intraoperative
monitoring, cerebral protection, anticoagulation therapy, and intraluminal
shunting, are emphasized. Operative techniques are discussed and illus¬
trated in detail.

Endovascular Therapy for the Carotid Artery 957

Endovascular therapy is well established as the preferred method of treat¬
ment for surgically inaccessible lesions of the carotid artery. Its role in the
treatment for cavernous carotid fistulas and aneurysms of the cervical,
petrous, and cavernous carotid arteries is discussed. Angioplasty is under
investigation in the management of symptomatic atherosclerotic disease.
The evolving role of angioplasty and stenting in the treatment of these
lesions is reviewed.
