The interface of bone and the endosseous implant surface has been de¬
scribed extensively in the literature. As a greater appreciation of the
function and esthetics of peri implant soft tissues has developed, the
effects of implant biomaterials on the transmucosal region have become
increasingly important. The structure of peri implant soft tissues is some¬
what similar to analogous periodontal structures, but several important
differences exist. The effects of implant surface composition, morphology,
and preparation procedures on surrounding soft tissue are not understood
completely but can be critical to the long term maintenance of a healthy
and esthetic soft tissue seal.

The success of long term implants depends on the health of the sur¬
rounding soft tissue. The authors discuss various soft tissue procedures
used to improve the predictability of an implant supported prosthesis as
well as the cause of soft tissue complications and their treatment. Oral
hygiene for endosseous implants and instrumentation also are reviewed.

Laser energy can be used to incise gingiva and mucosa around dental
implants. The clinical application of a specific type of laser depends on
the medium that is used to produce a specific wavelength of light. The
carbon dioxide laser is the laser of choice for soft tissue use around dental
implants because it targets the water molecule in the peri implant gingival
and mucosal tissues and only lases about 0.1 mm into the tissue.

The selection, planning, and execution of surgical procedures for anterior
dental implants require an understanding of the gross anatomy and histol¬
ogy of the structures and of the physiology and wound healing behaviors
of the tissues. A competent surgeon must be able to apply the general
biologic principles to unique clinical situations to bring about the desired
surgical outcome. In oral soft tissue grafting, the information that dictates
the type of epithelium that eventually will cover the surface of the recipi¬
ent site is found in the underlying connective tissue. Reestablishment and
maintenance of adequate blood supply to the graft are the main determi¬
nants of graft survival. A system of classifying the procedures of oral
soft tissue grafting according to the provision of their blood supply is
introduced.

Dense connective tissues are needed around dental implants in areas of
esthetic concern. This article details methods and timing for gaining addi¬
tional amounts of these soft tissues. These methods include the use of
connective tissues with or without an epithelial covering. Gaining addi¬
tional soft tissues is most predictable when performed before or concomi¬
tant with the implant placement.

This article discusses current prosthodontic protocol to ensure optimum
esthetic and functional results for implant supported restorations. The
critical aspects of the diagnostic work up are reviewed with an emphasis
on evaluation of the proposed implant site based on the planned restor¬
ative design. The role of the diagnostic wax up in the development of
radiographic and surgical guides to precisely place the implants as well
as to guide bone grafting procedures is emphasized. Finally, the types of
prosthetic abutments available are reviewed with particular attention on
optimizing the esthetic emergence profile of the final restoration.

planning and surgical procedures used during implant surgery to optimize
esthetic outcome.

The esthetic outcome of implant restoration depends on the emergence
profile or manner in which the restoration emerges through the soft tissue
from its implant base. The emergence profile should be visualized in the
final restoration before the initiation of therapy. This process is accom¬
plished jointly by the surgeon and restorative dentist, each of whom
shares the responsibility for the final successful case. Development of the

proper emergence profile then is addressed as a continuum of processes
throughout the course of implant therapy.

The need for bone grafting in cases of implant restoration depends on
several variables, including the tooth to be restored, the amount of avail¬
able bone, the type of implant to be used, and the timing of implant
placement. This article applies to implant restorations for the partially
edentulous patient and discusses ideal placement protocol for restoring
the central incisor and mandibular first molar—the two most challenging
teeth to restore with implants. The three common types of grafting materi¬
als also are discussed along with specifics about when they should be
used to restore various defects before implant placement, how and when
they should be placed, how they heal, and when guided bone regeneration
is advisable.

The restoration of esthetic implants in the anterior maxilla is challenging
to manage, especially in patients with a high lip line. Soft tissue manage¬
ment is initiated at the time of fixture positioning aided by specially
designed components. The creation of a papilla like formation between
fixtures and teeth is one of the last frontiers to be addressed in esthetic
implant dentistry.
