Identifying patients at highest risk for cardiac complications during non
cardiac surgery requires use of clinical stratification and, under special
circumstances, specialized cardiac testing. Active clinical conditions should
be stabilized, and the surgical team should have guidance in managing
cardiac medications and perioperative cardiac monitoring and in postoper¬
ative management of any cardiac complications. The impact of interven¬
tions to preempt cardiac complications is largely unstudied. When consid¬
ering coronary revascularization prior to elective noncardiac surgery, the
clinician should consider whether revascularization would be advised if
the patient were not undergoing the planned surgery, and the relative
urgency of the noncardiac surgery.

Pregnancy is associated with major hemodynamic changes in the cardio¬
vascular system that can contribute to greater morbidity and mortality
in women with underlying heart disease. Therefore, the diagnosis and
management of these disorders in the pregnant patient require under¬
standing of cardiovascular physiology during pregnancy, labor, delivery,
and the puerperium. It is also essential to have knowledge about safety
and utility of various drugs during pregnancy to treat maternal heart
disease without compromising fetal well being.

This article summarizes the role of the cardiac consultant in the manage¬
ment of patients with a broad range of neurologic problems. It emphasizes

the importance of anticoagulation in stroke prevention and of the recogni¬
tion of electrocardiogram abnormalities which may reflect a predomi¬
nantly neurologic process. A schema is presented that elucidates the mech¬
anisms involved in the myocardial manifestations of central nervous
system disease. Additionally, the myocardial involvement associated with
neuromuscular diseases is presented, emphasizing the major roles of heart
failure, arrhythmias, and sudden death in several of these diseases.

Cardiac complications are the most common causes of death in the end
stage renal failure population. The most commonly encountered cardiovas¬
cular conditions in this population are reviewed. Emphasis is given to the
pathophysiology and the practical management of these disorders.

Patients with mitral valve prolapse, chest pain with normal coronary
arteries, and neurologic syndromes following cardiac surgery constitute a
group that may present atypically, thus posing problems in management.
The management of patients with cardiac disease receiving electroconvul
sive therapy and of those on psychotropic drugs is also different from that
of the usual cardiology patient seen in the hospital. Clinical approaches
are suggested for the evaluation and treatment of each of these specific
conditions.

Sepsis and septic shock are significant contributors to in hospital morbid¬
ity and mortality. Use of a physiologic approach to the cardiovascular
abnormalities that characterize the septic state is essential for optimal
patient salvage. Administration of fluids, transfusion of blood products,
respiratory assistance, and the use of cardiotonic agents should be titrated
to specific end points of peripheral oxygen supply and demand.

A variety of environmental injuries may result in serious cardiovascular
disturbances. Although these conditions are infrequently encountered,
they may have devastating effects on the individual patient. The cardiovas¬
cular consequences of environmental injury may range from the subtle to
the dramatic. This article summarizes the pathophysiology and manage¬
ment of injuries resulting from burns, contact with domestic or natural
electricity, and exposure to extremes of atmospheric pressure.
